There are concrete jersey barriers protecting the wellhead and production equipment from vehicles. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. The soil stockpile is well stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. Preliminary restoration is complete.

1126585790 2014-07-07 Drill / Deepen / Reopen No Violations I was on location to inspect the progress of Nomac #72 moving in and scout out the location. Only light loads have bee transported at this point. The rig mats and containment are in place. No violations.
1130525078 2014-07-11 Casing String Witnessed No Violations I was on location to witness the cementing of the 13.375 surface casing. Nomac #72 drilled a 17.5 hole to a depth of 885 and Wyoming Casing ran 875 of 54.5lb/ft. casing into the hole. Schlumberger cement the casing with 167 barrels of 15.6lb/gal SurfaceSET-b. 46 barrels of cement circulated to surface during the job. See cement report for more details on this job. No violations.
1406638089 2014-07-03 Casing String Witnessed No Violations I witnessed the cementing of the conductor casing. Rocky Mountain Drilling drilled a 26 diameter hole to 120. Rocky Mountain installed 120 of 20 OD casing. Universal Well Services was the cementing contractor. The cement was mudscaled, and Universal pumped 52.4 bbls of cement slurry at 15.6 pounds per gallon. The cement was displaced with 39 bbls of water. 11 bbls of cement returned to surface. Displacement was completed at 11:26am.
1417554415 2014-07-28 Drill / Deepen / Reopen No Violations I inspected the location following the rig move of Nomac #72 to the Pidgeon pad. The location is in good condition and there is no drilling equipment on location. The containment and rig mats have been removed. There is a bull dozer and a fork lift on location. No violations.
2024129011 2015-11-23 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of three, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:


Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.
