0168686607 2015-12-04 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is equipped with a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

0712407377 2014-06-05 Production Wells No Violations Location status check. All equipment has been moved from location. All wells are gated and secured. The 8H & 3H are shut-in and this well and the 1H, 5H, & 10H have yet to be completed. The tank battery is diked and contains 6-400 barrel tanks. There is one vertical separator, 2 gas production units (GPUs), and one vapor recovery unit (VRU) on location. The location is in good condition, all IDs are posted at the entrance, and both gates at the entrance were properly locked. No violations.
1213648171 2014-05-15 Casing String Witnessed No Violations I returned to location to witness the 9.625 intermediate casing cement job. O-Tex cemented 2297 of casing with 179 barrels of Class H cement. 36 barrels of good cement circulated to surface. No violations. See casing report for more details on this job.
1228198343 2014-05-04 Casing String Witnessed No Violations I was on location at 7:54am to witness the cementing of the 20 conductor casing. Precision Rat Hole augered a 26 hole to 120 measured from ground level and the casing was butt welded and set open ended on bottom. Allied Cementing rigged up to the casing, established circulation, and mixed and pumped 250 sacks of Class A cement. The cement was displaced to 110 and 16 barrels of cement circulated to surface. 1413383215 2014-05-13 Casing String Witnessed No Violations I was on location at noon to witness the cementing of the 13.375 surface casing. O-Tex Pumping cemented 755 of surface casing with 148 barrels of Class H cement and 27 barrels of slurry circulated to surface. The plug did not land after pumping 3 barrels over displacement (113bbls total) and the floats held. The casing was pressure tested at 1000psi prior to drilling out the shoe track and satifies OAC 1501:9-1-08(D)3(b). No violations. See casing ticket for more details on this job.
1415127412 2014-05-22 Drill / Deepen / Reopen No Violations I was on location to check the drilling status of Nomac Rig #73. They were currently drilling the horizontal leg of the well at a measured depth of 14,594 with synthetic-oil based mud (SOBM). The site is in good condition, all equipment is on containment, and all fluids are circulating/contained in steel tanks. No violations.
1448549899 2014-05-15 Casing String Witnessed No Violations I was on location for the 9.625 intermediate casing cement job. Wyoming Casing was currently running pipe. I inspected the casing and it was new, 40lb/ft., grade J-55. Nomac Rig #73 had drilled a 12.25 hole to a depth of 2310. I left location to inspect Nomac Rig #72 at the Saltsman #10H and will return shortly. No violations.

I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The #10H well is in production and is equipped with a plunger-lift assembly. The production tanks are surrounded by a steel dike. The equipment on the pad consists of the following: 6 producing wells

1 Low pressure vertical separator 1 heat treater 6 sand separators 6 gas processing units 1 combustor unit
