-based mud used to drill and condition the hole, a lead cement followed by a tail cement were pumped. The Mudpush spacer was mudscaled, and pumped at 12.5 pounds per gallon (ppg). Schlumberger then pumped 287 bbls of lead cement at 13.5 ppg, and 285 bbls of tail cement at 15.0 ppg. The plug was dropped and displaced with 280 bbls of water as measured by pump truck tanks. The plug bumped the float collar at 11:50pm on 04/13/2014. The plug landing pressure was 2415 psi, and it was raised to 3350 psi, and held for 5 minutes. Pressure held. The float held when the pressure was released, and 3.5 bbls of water bled back. 38 bbls of spacer returned to the surface.

0574193211 2015-11-10 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is equipped with a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1042969166 2017-09-18 Production Wells No Violations I conducted a status check of the well while I was on site during plugging operations of the Kinsey #8 well (34-019-2-2538-0000) which was only drilled to conductor depth and never put in production. The #5H is the only producing well on the pad. The well is equipped with a plunger lift assembly. The equipment on the pad consists of the following:

6 - 400 bbl production tanks, three labelled as containing produced water, and three labelled as containing condensate. 1 sand separator 1 gas processing unit 1 combustor unit 1 heat treater No violations were noted at the time of inspection.

1264495427 2014-02-10 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 20 inch conductor casing. My inspection revealed that Precision Rathole has drilled a 26 inch hole to a total depth of 120 feet and ran 120 feet of 20 inch casing (94 pounds per foot / inside diameter is 19.124 inches). Allied cemented the conductor casing with 278 sacks of Class A 2% CaCl and 1/2 pound per sack floseal. The cement had a density of 15.6 pounds per gallon, a yield of 1.18 cubic feet per sack, and they used 5.23 gallons of mix water per sack. Allied pumped 58.4 barrels of cement, 39 barrels of displacement, and there was 5 barrels of cement returns to surface. My inspection revealed no violations at this time.
1397570913 2014-04-04 Drill / Deepen / Reopen No Violations I was on location witness the cementing of 13.375 conductor casing. Schlumberger pumped 25 barrels of Gel Spacer followed by 10 barrels of water. They pumped 636 sack of Class A cement. Displace with 75 barrels of water. 33 barrels of cement returned to surface. Plug landed at 300 psi. bumped plug to 800 psi and held for 5 minutes.
1397837201 2014-04-05 Casing String Witnessed No Violations
2066035287 2017-09-21 Production Wells No Violations I conducted a status check of this well while on site to conduct a final restoration inspection of the Kinsey #8 well (PN# 22538) which was drilled to conductor depth and plugged on 09/18/2017. The tin horn of the cellar belonging to the #8 well was removed and filled with gravel. No other changes were made to the well or pad since my last inspection. No violations were noted at the time of inspection.
