0581951608 2015-09-14 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly marked well identification sign. The tank battery consisted of eight 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock is installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

0653922740 2018-10-16 Production Wells No Violations The well is equipped with a plunger-lift assembly. No changes have been made to the pad production equipment since my last inspection. The surface/intermediate annular gauge displayed zero psi and the intermediate/production annular gauge displayed 480 psi. No violations were observed at the time of inspection.
1050110370 2017-11-16 Production Wells No Violations I conducted a status check of the well and pad. The pad houses six producing wells, and the well is equipped with a plunger lift assembly. The surface-

No violations were observed at the time of inspection.
1394281119 2014-03-08 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 13 3/8 inch surface casing. My inspection revealed that Nomac Drilling Rig # 73 has drilled a 17 1/5 hole to a total depth of 545 feet and they ran 535 feet of 13 3/8 inch surface casing (54.5 pounds per foot / inside diameter 12.615 inches). They installed 7 bowspring centralizers on the casing string; 2 on the first joint and then on 3,6,9,11, and 12. O-TEX cemented the casing with 500 sacks of Class H 2% CaCl, 0.4% C-41P, and 1/4 pound per sack celloflake. The cement had a density of 15.6 pounds per gallon, a yield of 1.2 cubic feet per sack, and they used 5.25 gallons of mix water per sack. They pumped 106.86 barrels of cement, 76.3 barrels of displacement, and there was 21 barrels of cement returns to surface. They bumped the plug with 380 PSI and then conducted the casing test with 935 PSI. The casing test was approved, floats held when the pressure was released, and there was approximately 1/2 barrel of water flowed back. My inspection revealed no violations at this time.
1394426163 2014-03-10 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 9 5/8 inch intermediate casing. My inspection revealed that Nomac Drilling Rig # 73 has drilled a 12 1/4 inch hole to a total depth of 1927 feet and ran 1917 feet of 9 5/8 inch intermediate casing (40 pounds per foot / inside diameter is 8.835 inches). They installed 15 bowspring centralizers on the casing string, on joints 1,3,6,9,12,15,18,21,24,27,30,33,36,39, and 42. O-TEX cemented the casing with 710 sacks of Class H 3% Gypsum, 2% CaCl, 0.8% MC-500, 0.5% C-45, and 0.4% C-41P. The cement had a density of 15.8 pounds per gallon, a yield of 1.21 cubic feet per sack, and they used 5.14 gallons of mix water per sack. O-TEX pumped 153 barrels of cement slurry, 138.5 barrels of displacement, and there was 33 barrels of cement returns to surface. They bumped the plug with 736 PSI and then conducted the casing test with 1185 PSI. The casing test was approved, pressure released, floats held, and there was approximately 1 barrel of water flowed back. My inspection revealed no violations at this time.
1428486147 2013-12-24 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 20 inch conductor casing. My inspection revealed that Express has drilled a 26 inch hole to a total depth of 120 feet and ran 120 feet of 20 inch conductor casing (94 pounds per foot / inside diameter 19.124 inches). Allied cemented the casing with 250 sacks of Class A with 2% CaCl and 1/2 pound per sack flake. The cement had a density of 15.6 pounds per gallon, a yield of 1.18 cubic feet per sack, and they used 5.23 gallons of mix water per sack. Allied pumped 52.5 barrels of cement, pumped 39 barrels of displacement, and there was 5 barrels of cement returns to surface. My inspection revealed no violations at this time.
