0039441185 2015-01-15 Production Wells No Violations I was on location to conduct a status check of this well. The well is open flow with a wellhead tree assembly. Jersey barriers were installed around the wellhead. The tank battery consisted of 2, 400 BBL production tanks and was diked. A windsock was installed on the top of the production tanks. Other production equipment on the well pad consisted of:


Jersey barriers are installed around the production equipment. Lightening protection is installed on top of the production tanks. The well is properly identified with a clear, legibly marked well identification sign. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. The lease road is stable and drivable. No violations found.

There are concrete jersey barriers protecting the wellhead and production equipment from vehicles. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. The soil stockpile is well stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. Preliminary restoration is complete.

0182183315 2013-10-13 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 20 inch conductor casing. My inspection revealed that Precision has drilled a 26 inch hole to a total depth of 120 feet and ran 115 feet of 20 inch casing (94 pound per foot/ inside diameter 19.124 inches) into the well bore. Allied cemented the casing with 250 sacks of ClassA 2% CaCl and 1/2 pound per sack flake. The cement had a density of 15.6, a yield of 1.2 cubic feet per sack, and they used 5.25 gallons of mix water per sack. They pumped 53.4 barrels of cement slurry, 39 barrels of freshwater displacement, and there was 11 barrels of cement returns to surface. My inspection revealed no violations at this time.
0650539263 2017-09-14 Production Wells No Violations I conducted a status check of the well while I was on site during plugging operations of the Lafferty #1H well (34-019-2-2504-0000) which was only drilled to conductor depth and never put in production. The #6H is the only producing well on the pad, and no other changes have been made to the pad since my last inspection. No violations were noted at the time of inspection.
1624836396 2017-09-18 Production Wells No Violations I conducted a status check of the well while performing a final restoration inspection of the Lafferty #1H well (34-019-2-2504-00-00) which was plugged on 09/14/2017. The #6H well is the only producing well on the pad. No changes have been made since my last inspection, and no violations were noted.

Identification signs were posted at the access gate, the well, the GPU, and the tank battery. The #6H well is in production and is equipped with a plunger lift assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and all man doors were locked.

The equipment on the pad consists of the following: 2- 400 bbl production tanks; both labelled as containing produced water. 1 Producing well 1 Sand separator 1 gas processing unit

Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly marked well identification sign. The tank battery consisted of two 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock is installed on top of the production tanks. Other production equipment on the well pad consisted of:

Along the interior of the chain link fence there is an earthen berm and it was stable. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.
