0046936060 2018-10-29 Production Wells No Violations The well was equipped with a plunger-lift assembly. The surface-intermediate annular gauge displayed zero psi, and the intermediate-production annular gauge displayed 130 psi. I performed a walk-around inspection of the pad. Erosional controls appeared in place and functioning. No violations were observed at the time of inspection.
0987179865 2018-06-21 Production Wells No Violations The well is equipped with a plunger-lift unit. The surface-intermediate annular pressure gauge displayed a reading of approximately 100 psi, and the intermediate-production annular gauge displayed zero psi. No changes to the pad since my last inspection. No violations were observed.
1377626720 2013-08-25 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 20 conductor casing (94 pound per foot and inside diameter 19.124 inches). My inspection revealed that Butchs Rathole had drilled a 30 inch hole to a depth of 120 feet and ran 120 feet of 20 inch conductor casing into the well bore. Allied cemented the casing with 462 sacks of Class A ( 2% CaCl and 1/2 pound per sack flake) with a 1.18 yield. They pumped 97 barrels (15.6 pounds per gallon density)of cement slurry, displaced with 39 barrels of freshwater, and got 22 barrels of cement returns to surface. No violations at this time. NOTE: Spudded well on 08/23/2013.
1409828704 2014-08-14 Production Wells No Violations The Uniontown office received a complaint call from Rebecca Thompson ( 2093 Moccasin Rd. Carrollton, Oh 44615 ph.-- 330.340.9003 ) stating that she was smelling a odor that she thought was originating from the Hutchison Pad ( 2160 Moccasin Rd. Carrollton , Oh. 44615 ). Mrs. Thompson contacted Chesapeake Exploration to register her concern about the odor. She said she left her home out of concern for her family. Chesapeake told her that they responded to the Hutchison Pad and did not find odors or emissions from the pad. Upon her return the smell was not present but returned a few hours later. Inspector Bob Roush visited the Hutchison Pad and could not detect any odor or gas smells. There are 3 producing wells on the pad, Hutchison #1-H, #3-H, #5-H. The vapor recovery system was operating properly and all the other production equipment was leak - free . Kurt Kollar ( OEPA ) responded to Mrs. Thompsons call to the OEPA. Mr. Kollar concurred that the Hutchison Pad was not the source of the odor. I visited with Mrs. Thompson and relayed our findings that all appeared to be in order and leak -free. On 8/15/2014 I spoke with Kurt Kollar ( OEPA ) who said that the source of the odor was from American Landfill in Stark County. He had received a group of complaints describing the same odor that Mrs. Thompson had noticed. The complaint plume originated in Stark County and moved easterly and into Carroll County. Kollar said a zinc alkyloid dye had mixed with some water and created the emission that was noticed. Kollar also stated that the smell was not related to Chesapeakes producing operations. Complaint # 4775 is closed.
1418234180 2014-08-13 Production Wells No Violations I was on location with Inspector Bob Roush and Kurt Kollar (OEPA) to investigate if there was an ammoniated odor emanating from this well pad that would affect a residence to the west. We found nothing on location that could generate an ammonia odor and the vapor recovery unit (VRU) was functioning properly. It was later discovered that a landfill in East Canton was the source of the odor. Bob Roush is handling the complaint. This is not oil & gas related. No violations.
1870904473 2017-06-19 Preliminary Restoration No Violations

I was on location to conduct a preliminary restoration inspection. The well is equipped with a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a legibly-marked well identification sign. Identification was posted at the tank battery which consisted of eight 400 bbl production tanks, three labeled as containing condensate, and five labelled as containing produced water, and one low-pressure vertical separator. The tank battery is surrounded by a dike and the tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of: 8 -400 bbl tanks, three labelled as containing fresh water and five labelled as containing condensate 3 producing wells 3 sand separators 3 gas processing units 1 vapor recovery unit 1 heat treater

Jersey barriers are installed around the production equipment.  The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire.  The  man doors were locked.   Drainage and erosion controls at the pad are in place and functioning.  A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure.  The pad has a crushed limestone surface, and is in good repair.  The lease road was stable in in good repair.
