0797721868 2014-11-05 Casing String Witnessed No Violations I was on location to witness the cementing of the 9.625 intermediate casing. Nomac Rig #43 drilled a 12.25 intermediate hole to a depth of 1675 and Wyoming Casing ran 1660 of 40lb/ft. casing into the hole. Schlumberger rigged up and cemented the casing with 136 barrels of D901 (Class A) cement slurry. 20 barrels of cement weighing 15.0lbs/gal on the pressurized scales circulated to the cellar. 1332667787 2014-05-01 Casing String Witnessed No Violations I was on location at 1:14pm to witness the cementing of the 20 conductor casing. Enviro had augured a 26 hole to a depth of 120 measured from ground level (GL) on 4/30/14. 20 conductor casing was butt welded and set on bottom with holes cut at 3 and 6 off bottom to allow circulation. A 20 steel plate with a 2 ball valve was welded to the top of the casing. Allied Oil & Gas Services rigged up to the 2 valve, tested the lines, and pumped a 5 barrel water spacer to establish circulation. They then mixed and pumped 247 sacks of cement and displaced the cement to 110 with 39 barrels of water. 15 barrels of cement circulated to the cellar and held. No violations. 1416329622 2014-10-04 Drill / Deepen / Reopen No Violations I performed a status check on the above location. At this time they are removing the BOP from under the rig and replacing and repairing the seal in the hydraulic rams. They will perform a BOP test when completed. Drilling permits and condition are posted. Location is clean.
1591661692 2015-12-10 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1600440753 2014-11-06 Drill / Deepen / Reopen No Violations I performed a status check on the above location. At present Nomac Rig is at a depth of 1600. All permits and conditions are posted in the company trailer. The location is clean. No noted violations at this time.
1707285131 2016-02-22 Production Wells No Violations I was on location for a status check. My inspection found the access road gate is locked and the access road is in good repair. Identification signs are posted at the access road entrance, well and storage tanks. The pad is fenced and all man doors were locked. The #1H is in production. The well is equipped with a christmas tree valve assembly. There is a steel dike around the storage tanks.

The equipment on the pad consists of the following: 6 Storage tanks 2 Sand separators 2 Gas processing units (GPU) 1 Vertical separator 1 Vapor recovery unit (VRU) 1 Thermal electric generator (TEG) 2 Production wells on the pad 1 Heat Treater Site fenced and man doors locked
