0328086295 2013-07-03 Casing String Witnessed No Violations I was onsite to witness the cementing of the 13 3/8 surface casing set to 575 feet for this well. The rig was down for several days before drilling continued again on this well. The job was pumped as planned and the cement was weighed in at 15.6ppg before it was pumped down hole. There were a total of 29 barrels of cement returned to surface. There are no violations at this time.
0816517274 2013-06-20 Not Drilled No Violations I was inspecting this location to determine how much longer construction has before the location can be completed. Nomac 72 is going to be moving to this location when they completed the final well on the Putnam well pad. Currently an auger rig is on location drilling conductors for the location. I was not able to access the pad due to a very large number of slag trucks waiting to access the pad. Erosion controls have been installed around the location but growth has not been established yet. It appears the location is still several days away from being completed. There are no violations at this time.
0827560966 2013-07-15 Casing String Witnessed No Violations I was onsite to witness the cementing of the 5.5 production casing drilled by Nomac 72. The casing was run to a depth of 15806. The crews had issues getting the casing rotating tool rigged up and there were 2 leaks during the job. The first leak was in the stack during the pressure test and the second leak was in the iron near the cement head during the pumping of the lead cement weighed in at 13.5ppg. The job was completed and the top of cement is estimated at 535. They will now walk the rig to the 8H well.
0901099431 2013-07-11 Administrative Inspection No Violations I contacted Nomac 72 to check if there was any structural damage down to the wells following a severe storm that came through the area, there was no damage. Currently the rig is broken down due to generator issues. They are currently at 10750 of there 15500 of drilling for this well.
1040033310 2017-03-01 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The access road was equipped with a gate and was locked. The #6H well is in production and is equipped with an open flow assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and all man doors were locked.

The equipment on the pad consists of the following: 4 producing wells

1 Low pressure vertical separator located inside of dike 4 sand separators 4 gas processing units 1 vapor recovery unit 1 heat treater 1 compressor station

No violations were found during my inspection.

1372174346 2013-06-24 Drill / Deepen / Reopen No Violations I was onsite to check the status of the well pad completion. Nomac 72 will be coming to this location following the completion of the Putnam wells. Currently the conductor is drilled for this well and is awaiting the rig. Erosion controls have been put into place but the vegetation has not yet started to grow. Slag piles are still on the pad but will be put into place today.
1372803294 2013-07-01 Drill / Deepen / Reopen No Violations I was onsite to check the status of Nomac 72 drilling after being moved from the Putnam pad. Currently the rig has been down for 62 hours after drilling 80 feet. The guides for the top drive were bent during the rig down and move to the new location. They attempted to center the top drive but were unable to. They had to rig down the top drive and remove the guide so that it could be sent to the Nomac yard in West Virginia to be worked on. They expect to it return Tuesday night and they will continue drilling ahead at that time. They are going to drill ahead 570 and log the hole before they run the 13 3/8. Currently the crews are cleaning and pressure washing the rig during the down time. Several compressors for drilling were also being brought on location at this time.
1373921879 2013-07-14 Administrative Inspection No Violations I was contacted by rig Nomac 72 informing me that they have reached TD and will start pulling out of the hole. They expect to cement 5.5 production casing tomorrow at noon.
1706855516 2015-10-23 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

2121109199 2013-07-12 Drill / Deepen / Reopen No Violations I was onsite to check the status of Nomac 72 drilling ahead in the lateral. They were at a depth of 12833 and are currently tripping out of the hole due to the failure of the mud motor. They were currently pumping the slug to trip our after performing a cleanup cycle on the well. I also checked the locations erosion controls following a strong storm. New work has been done on the controls and there are no violations at this time.
