0012008558 2018-04-26 Production Wells No Violations I conducted a status check of the well and pad. No violations were observed at the time of inspection. The surface/intermediate displayed a reading of zero psi and the intermediate/production annular gauges displayed a reading 40 psi.
0128363864 2015-01-15 Production Wells No Violations I was on location to conduct a status check of this well. The well is open flow with a wellhead tree assembly. Jersey barriers were installed around the wellhead. The tank battery consisted of 6, 400 BBL production tanks and was diked. A windsock was installed on the top of the production tanks. Other production equipment on the well pad consisted of:

Jersey barriers are installed around the production equipment. Lightening protection is installed on top of the production tanks. The well is properly identified with a clear, legibly marked well identification sign. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. The lease road is stable and drivable. No violations found.

0892762329 2016-03-10 Preliminary Restoration No Violations

Production equipment on pad includes:

There are concrete jersey barriers protecting the wellhead and production equipment from vehicles. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. The soil stockpile is well stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. Preliminary restoration is complete.

1856748877 2017-09-06 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The access road was equipped with a gate and was locked. The #10H well is in production. The well is equipped with a plunger lift assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and all man doors were locked.

The equipment on the pad consists of the following: 6- 400 bbl production tanks; three labelled as containing produced water, and three labelled as containing condensate. 3 Producing wells 3 Sand separators 3 Gas processing units

Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.
