0989081361 2018-11-19 Production Wells No Violations The well was equipped with a plunger-lift assembly. The surface/intermediate and intermediate/production annular gauges displayed readings of zero psi. I performed a walk-around inspection of the pad and erosional controls appeared functioning. No violations were observed at the time of inspection.
1394784868 2014-03-14 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 13 3/8 inch surface casing. My inspection revealed that Nomac Drilling Rig # 71 has drilled a 17 1/2 inch hole to a total depth of 575 feet and ran 562 feet of 13 3/8 surface casing (54.5 pounds per foot / inside diamter is 12.615 inches). They installed 6 bowspring centralizers on the casing string on joints 1,2,3,6,9, and 12. Schlumberger cemented the casing with 515 sacks of D901 (Class A) 1% D154, 0.5% B348, 0.15% B160A, 0.2% D046, 0.75% S001, and .125 pound per sack D130. The cement had a density of 15.6 pounds per gallon, a yield of 1.2 cubic feet per sack, and they used 5.219 gallons of mix water per sack. Schlumberger pumped 110 barrels of cement slurry, 80 barrels of displacement, and there was 30 barrels of cement returns to surface. They bumped the plug with 260 PSI and then conducted the casing test with 1110 PSI. The casing test was approved, pressure released, floats held, and the was 1/2 barrel of water flowed back. My inspection revealed no violations at this time.
1396977284 2014-04-08 Drill / Deepen / Reopen No Violations A general inspection of the Ray Haley pad, well number 10 H revealed that the site is in good condition with some standing water. Nomac 71 is the drilling contractor and is coming out of the hole at seven thousand feet. They will begin laying down drill steel and start running production casing in twenty four to thirty hours. No violations found. I spoke with Terry, the company man; he stated that there have been no spills or accidents to report. He did state that in the last ten days they have had trouble getting locked in the hole.
1413376110 2014-02-03 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 20 inch conductor casing. My inspection revealed that Enviro Drill has drilled a 30 inch hole to a total depth of 120 feet and ran 120 feet of 20 inch conductor casing (78.67 pounds per foot / inside diameter of 19.25 inches). Allied pumped 433 sacks of Class A cement with 2% CaCl and 1/2 pound per sack floseal. The cement had a density of 15.6 pounds per gallon, a yield of 1.18 cubic feet per sack, and they used 5.23 gallons of mix water per sack. Allied pumped 91 barrels of cement slurry, 39.5 barrels of displacement, and there was 2 barrels of cement returns to surface. Prior to cementing the casing, Allied bulk truck broke down and they couldnt air-up the canisters on the bulk truck. Allied sent a truck to their Cambridge yard to get another compressor to air-up the canisters on the bulk truck. My inspection revealed no violations at this time. NOTE: Allied mixed 150 pounds of gel and pumped it ahead of the cement because it appeared that there was communication with the 5H conductor hole.
1803156646 2017-03-02 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The #10H well is in production and is equipped with an open flow assembly. Jersey barriers were installed around the wellhead. The well is identified with a legibly-marked well identification sign. Identification was posted at the tank battery which consisted of eight 400 bbl production tanks, four labeled as containing produced water, and four labelled as containing condensate, and one low-pressure vertical separator. The tank battery is surrounded by a dike. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Production equipment on the well pad consisted of:

6 producing wells 8- 400 bbl production tanks, four labelled as containing fresh water, and four labelled as containing condensate 6 sand separators 6 gas processing units 1 low pressure vertical separator located inside of the dike 1 vapor recovery unit 1 horizontal separator, 1 compressor station 2 bullet tanks Jersey barriers are installed around the production equipment. The access road and man doors were locked. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road was stable in in good repair.
