0129368331 2013-04-15 Drill / Deepen / Reopen No Violations I was onsite to check back with the status of the leaking wells on the pad location. A senior well tender checked the site and is still stating that the bubbling is trapped air stuck in the annulus of the casing strings. Rex informed me that they are going to bring out the wellhead company to tighten down the bolts and wellhead. I let them know that they need to vent the air and then install pressure gauges on the annulus to monitor the wells before they can be prepared to be hydraulically fractured. An additional gas meter reading still showed no signs of any methane or explosive/dangerous gases.
0499436029 2013-03-05 Drill / Deepen / Reopen No Violations I was onsite to witness the cementing of the 13 3/8 surface casing set at 1324 feet. The job was completed with full returns through the job. A total of 60 barrels of cement returned to surface. The location is in good shape and there are no violations at this time.

The 2 wells on location have been drilled, covered and completed. There are no violation at this location.
1167696472 2013-06-12 Production Wells No Violations I was onsite to check the status of these wells being put into production. The production facilities are complete and the wells, with complete wellheads, are being welded into the flow lines. The wells should be in production soon.
1363201780 2013-03-12 Drill / Deepen / Reopen No Violations I was onsite to witness the 2 stage cementing of the 9 5/8 intermediate casing set at 6254 feet. The first stage went as planned and the bomb was dropped to the DV tool allowing the ports to properly open. The 2nd stage was circulated for 6 hours allowing the excess cement to removed and the 1st stage cement to set. The 2nd stage commenced as planned and there was a total of 65 barrels of cement returned to surface.
1363805291 2013-03-19 Drill / Deepen / Reopen No Violations I was onsite to check the status of UDI drilling rig. They currently just got off their curve and are drilling ahead at 9000. They plan on hitting their TD on Saturday. There are no violations at this location at this town.
1364482914 2013-03-22 Drill / Deepen / Reopen No Violations I was onsite to check the status of the cementing of the 5.5 production casing. Universal cementing was not onsite yet but the casing was run and the well was being circulated. I went over the cement plan with the company man before leaving for the Harvey 5H surface cement job.
1365704856 2013-04-11 Drill / Deepen / Reopen No Violations I was onsite to check the status of the drilling pad following the removal of the rig and drilling equipment. New containment has been put down around the 2 wells and the wells are capped and surrounded by steel fencing. 8 frac tanks are on location with new containment as well. The erosion controls are holding up well following a series of strong thunderstorms that moved through the area the previous night. The wells are expected to by hydraulically fractured starting 4-29-2013 but before they can be a possible leaking wellhead needs to be addressed. One of the well cellars is full of water and the submerged wellhead is bubbling gas, the other wellhead has a noticeable hiss of leaking gas/air. I have contacted Regional Supervisor Tom Hill and Rex Energy. Rex is having a senior completions manager check the location to determine what the cause of the bubbling is. A Rex safety man told me that the bubbling can be leaking air that is trapped in the well annulus. I informed Rex to have the cellars pumped empty, the leaks identified and pressure gauges placed on the wellheads to determine what kind of pressures are currently on the annuli to determine what the possible issues are. My gas meter showed no signs of dangerous gas when sampled.
1366388527 2013-04-17 Drill / Deepen / Reopen No Violations I was onsite with supervisor Tom Hill and inspector Brett Farnsworth to evaluate the situation of the leaking well heads at this location. The wellhead bolts have been tightened down again and the cellar has been pumped down. A well tender will be on location tomorrow at 9am to installed pressure gauges to monitor the wellhead pressures to see if the gas is from a formation or compressed air from thermal expansion.
1366649776 2013-04-22 Drill / Deepen / Reopen No Violations I was onsite to check the wellhead annular pressures to determine whether or not the well is static and that the casings are not building pressures before the wells are to be hydraulically fractured. Currently both wells are showing 0 psi on the casing annular spaces. Canary wellhead services is onsite assembling the wellheads for the frac operations that are to start in roughly on week. The wells will be continued to be monitored.
1367429817 2013-04-29 Drill / Deepen / Reopen No Violations I was onsite to witness the beginning of the hydraulic fracturing for both the Brace West 1H and 2H wells. FracTec is on location and preparing to frac the wells. Currently sand is being hauled on location as well as the chemicals for the frac job. The annular pressures of the wells are going to be constantly monitored during the job to ensure well integrity. A recent gas transmission line has also been installed on location for full production when the wells are completed.
1368112526 2013-05-08 Drill / Deepen / Reopen No Violations I was onsite to witness the fracking of this well and check the pressures of the annular spaces. They are currently fracking the 2H well and are on the 47th of 54 stages for this 2 well pad. This well currently has 20 psi of pressure on the 13 3/8 and 9 5/8 annular space and has not been building pressure. They expect to be done fracking these wells Friday and will spend several days prepping the well before giving it a rest period.
1412688396 2014-05-01 Production Wells No Violations I was on location to investigate complaint #4690 concerning possible vibrations caused by oil and gas activities. This pad is located 3/4 of a mile WNW of the Complainants property. My inspection found the Brace West #s 1H & 2H (PN# 22285) in production. The only equipment in operation was a small compressor and could not be the source of vibration. No violations.
1416679448 2013-07-15 Production Wells No Violations I was onsite to check the erosion controls of this wellsite following strong storms that made it through the region the previous week. There were no issues on the pad and all erosion controls are working properly. The wells on this site are currently not producing as they are waiting for the completion of the production facilities for the two wells. A crew is onsite production testing the wells with a flair stack set up. Rex gave these wells a shut in period to allow for supposed increased production.
1459506428 2013-02-28 Drill / Deepen / Reopen No Violations I was onsite to witness the cementing of the conductor for this well. They had issues with the original conductor that was cocentrically drilled. They removed that 20 pipe and went in to reem the hole to 24. They set 120 feet of 20 conductor and set it to surface. A calculated 3 barrels of cement returned to the surface into the cellar. There are no violations at this time.
1464913784 2013-03-07 Drill / Deepen / Reopen No Violations I was onsite to pickup the paperwork from the surface cement job. The rig is currently drilling the intermediate casing hole at 1780. Their target depth is around 6400 feet. There are no violations at this time.
1529596314 2018-06-21 Production Wells No Violations I conducted a status check of the well and production pad. The pad is equipped with two producing wells (permit numbers 22285 & 22284). This well is equipped a plunger-lift assembly. Identification was posted at the well and lease entrance. The production tanks, gas processing units, and separators were surrounded by a steel dike and was lined. Some areas inside of the dike contained less than on inch of standing water. I tested the standing water for chlorides using a low--around of the pad perimeter and erosion controls are in place and properly functioning.

Equipment on the pad consisted of the following:

The well was equipped with pressure relief valves on the surface-intermediate, and intermediate-production annuli. I did not locate a surface-intermediate annular pressure gauge. I contacted operator who stated they in the process of acquiring gauges from the supplier, and an annular gauge will be installed as soon as possible.

1797727194 2013-04-24 Drill / Deepen / Reopen No Violations I was on location to talk with the Rex Energy company man regarding the upcoming hydraulic fracturing of the Brace West wells. The annulus pressures are currently reading 0psi and are being monitored. Coiled tubing is being run into the wells at this time in preparation. The well annulus pressures are going to be electronically monitored throughout the fracking as a precaution. I am going to be onsite Monday to witness portions of the frack job.
1986417755 2013-05-14 Drill / Deepen / Reopen No Violations I was onsite to check the status of the competitions of these 2 wells on the location. They have completed the hydraulic fracturing of both wells and are currently drilling out the plugs in the 1H well. They completed all 54 stages while keeping the injection pressures below 10,000psi to ensure the possible issues with the cementing to not be an issue. They will continue to drill out the plugs on both wells and let them have a resting period before putting the wells into production. They estimate to be on location until at least Saturday.
