0633161518 2018-11-08 Production Wells No Violations The well was equipped with a plunger-lift assembly. The surface/intermediate annular gauge displayed a reading of zero psi, and the intermediate/production annular gauge displayed 600 psi. I performed a walk-around inspection of the production pad and erosional controls appeared functioning.
0920776855 2012-01-07 Drill / Deepen / Reopen No Violations On site to witness the cementing of the 20 inch conductor casing. My inspection revealed that Precision Rathole spudded the well on January 4th and ran the casing on January 5th. Precision drilled a 30 inch to a depth of 20 and then drilled a 24 inch hole to a depth of 116 feet. Precision ran 116 feet of 20 inch casing into the wellbore. O-TEX cemented the casing with 195 sacks of ClassA cement with 2% CaCl, .27% Cello-Flake, and 4/10% C-41P. The cement had a density of 15.6 pounds per gallon and a yield of 1.18 cubic feet per sack. They used 5.2 gallons of freshwater per sack mix water. O-TEX pumped 40.96 barrels of cement and displaced the cement with 39.4 barrels of freshwater. There was approximately 6 barrels of cement returns to surface. My inspection of the well site revealed no violations at this time.
1697581764 2015-11-16 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.
