0001733599 2017-02-01 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is equipped with a plunger-lift assembly. Te #2H well is in production. Jersey barriers were installed around the wellhead. The well is identified with a legibly-marked well identification sign. Identification was posted at the tank battery which consisted of eight 400 bbl production tanks, four labeled as containing produced water, and four labelled as containing condensate. The tank battery is surrounded by a dike. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Production equipment on the well pad consisted of:

2 producing wells 8- 400 bbl production tanks, four labelled as containing fresh water, and four labelled as containing condensate 2 sand separator 2 gas processing units 1 vapor recovery unit Jersey barriers are installed around the production equipment. The access road was locked Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The lease road was stable in in good repair.

0731706340 2018-06-27 Production Wells No Violations I conducted a status check of the well and production pad. Two wells are located on the pad (permit numbers 22133 & 22244. This well is equipped with a plunger-lift assembly. The surface--around inspection of the pad perimeter, and erosional controls appeared to be functioning.

Equipment on the pad consisted of the following: Eight 400 bbl production tanks; four labelled as containing condensate and four as containing produced water Two gas processing units Two vertical separators One combustor unit

1451420545 2018-10-29 Production Wells No Violations The well was equipped with a plunger-lift assembly. The surface-intermediate annular gauge displayed a reading 350 psi I performed a walk-around inspection of the pad and erosional controls appeared in place and functioning. I spoke with the operator regarding the annular pressure, and they stated that they will blow-down the well to lower the pressure and will monitor.
