0234304245 2015-12-30 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

0359126518 2018-11-01 Production Wells No Violations The well was equipped with a plunger-lift assembly. The surface/intermediate and intermediate/production annular gauges displayed zero psi. No violations were observed on the production pad at the time of inspection.

The tank battery is surrounded by a dike and the tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:

0787189679 2018-11-01 Production Wells No Violations The well was equipped with a plunger-lift assembly. The surface/intermediate and intermediate/production annular gauge displayed readings of zero psi. I performed a walk-around inspection of the pad and erosional controls appeared functioning. No violations were observed at the time of inspection.
1355105862 2012-12-09 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 13 3/8 surface casing. My inspection revealed that Patterson Rig # 261 has drilled a 17 1/2 hole to a total depth of 550. They ran 550 of 54.5 pound per foot 13 3/8 surface casing. Seven centralizers were installed on the casing string on joints 1,2,4,6,8,10,and 12. O-TEX cemented the casing with 640 sacks of Class H cement with 2% CaCl, .27% Cello Flake, and .4% C-41P. There was good circulation throughout the job and approximately 38 barrels of cement was circulated to surface. Pressure tested the casing at approximately 1040 psi for 5 minutes with no pressure loss; test approved. Inspection of the well site revealed no violations at this time. Note: Drilling rig had trouble with pumps being frozen and steel pits being filled to maximum capacity. The drilling rig had to wait on a roll off tank before O-TEX could conduct the cement job.
