0383643725 2013-02-28 Drill / Deepen / Reopen No Violations I was onsite to witness the cementing of the production casing for this well. The cement job went as planned. Following the cementing of this well the rig is going to skid to the next well on this pad location and should be spudding Sunday night. There are no violations at this location at this time.
0642698709 2013-04-15 Drill / Deepen / Reopen No Violations I was onsite to get updated GPS coordinates for the wells on the Harvey pad. There are 4 drilled wells on the pad, 1 well was previously drilled and is producing. There is production equipment onsite including a 6 400 barrel production tank battery surrounded by a steel dike. The wells appear to be static and the site is being prepared to be hydraulically fractured in the near future. A Grady Rentals crew was onsite laying down containment for the well pad. Surrounding the well pad is new erosion controls which have been recently installed. There are no site violations at this time.
1145205667 2013-07-15 Production Wells No Violations I was onsite to check the erosion controls of this wellsite following strong storms that made it through the region the previous week. There were no issues on the pad and all erosion controls are working properly. The wells on this site are currently shut-in as they are waiting for the completion of the production facilities for the four wells.
1319113528 2016-02-22 Production Wells No Violations I conducted a status check inspection of this Utica Shale well pad. My inspection found the lease road gate present and locked. The lease road and apron are in good repair. The pad is fenced and the entrance gate and all man doors on the perimeter of the pad were locked. The #1H well is in production, equipped with a plunger lift system. Well #s 3H (PN# 22301), 5H (PN# 22302), and 8H (PN# 22076-01) are also in production and producing with the aid of plunger lift systems. The proper IDs are posted at the entrance and at the wellheads and tank battery. There is a one (1) acre, freshwater impoundment located to the north of the pad. The impoundment is lined, fenced, locked and identified. The pad and impoundment area are equipped with video surveillance.

The equipment on the pad consists of the following:

6-400bbl storage tanks (3 water, 3 oil/condensate) 4 Producing wells 4 Sand separators

1 Oil stabilizer 1 Low pressure, vertical separator 2 Thermo-electric desiccators with a vertical drip bottle 1 Self-contained, 350 gallon, methanol tank containing Assure HI-18FB hydrate inhibitor 1 Combustor with inline pressure tank and air compressor

1358357053 2013-01-16 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 20 conductor casing. My inspection revealed that Express Energy Services was still welding the last joint of casing. Express has drilled a 26 hole to a total depth of 120. They ran 120 of 20 casing and the casing was set on bottom. Express Energy Services pumped 5 barrels of freshwater ahead and mixed 245 sacks of Class A cement (3% CaCl and .5 pound per sack floseal). The cement slurry had a density of 15.6 pounds per gallon and a yield of 1.18 cubic feet per sack. They pumped 51.49 barrels of cement slurry and displaced with 39.07 barrels of freshwater. There was good circulation throughout the job and approximately 13 barrels of cement was circulated to surface. No violations noted during this inspection.
1361980520 2013-02-26 Drill / Deepen / Reopen No Violations I was onsite to check the status of Nomac 72 drilling. They were currently at 13000 feet in the lateral of their current well. They expect to drill to total depth tonight. They will circulate the well, trip out of the hole and then begin to start running casing. There are no violations at this time.
1368492943 2013-05-13 Drill / Deepen / Reopen No Violations A workover rig was on location to prepare the wells for production following the fracturing of the wells. Production equipment is set up on location and pipelines are on the location for production of the wells. A crew member said that they would be working on location several more days until all the wells are completed, following this a crew will come onsite and install the wellheads for the wells.

1685465771 2013-06-06 Production Wells No Violations I was onsite to check the status of these wells. The wells have been drilled, hydraulically fractured and fully prepared for production. Currently crews are onsite preparing the flow lines for the wells to the production facilities onsite. The wells are currently shut in and the annulus and wellheads show not recordable pressures at this time. There is a gas sales line for this location so that gas will be sold when the wells are in production. There are no violations at this time for this location and these wells.
1793988944 2015-10-01 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly marked well identification sign. The tank battery consisted of six 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock is installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1910959499 2017-07-12 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access entrance, the tank battery, and the well. The #1H well is in production and is equipped with a plunger-lift assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and all man doors were locked.

The equipment on the pad consists of the following: 4 producing wells

1 Low pressure vertical separator 4 sand separators 4 gas processing units 1 vapor recovery unit 1 heat treater The access road was in good repair.
