0419947848 2012-08-14 Drill / Deepen / Reopen No Violations I was on site to check the status of Patterson Drilling Rig # 261. My inspection revealed that Patterson Rig # 261 is rigging up and should spud the 17.5 hole sometime tomorrow or early the next day. Dean Cook (company man) advised that he would call me when they were ready to spud the well.He also advised me that they are planning on setting approximately 550 of 13 3/8 surface casing. My inspection revealed no violations at this time.
0985980399 2018-11-19 Production Wells No Violations The well was equipped with a plunger-lift assembly. The surface/intermediate annular gauge displayed a reading of zero psi and the intermediate/production displayed 365 psi. I performed a walk-around inspection of the pad and no violations were observed at the time of inspection.
1341192892 2012-06-28 Drill / Deepen / Reopen No Violations My inspection revealed that Express Energy Services is drilling the 26 conductor hole. The driller advised that they were drilling at 105 and that the total depth of the conductor hole should be 120. He advised that they would run the 20 conductor pipe tomorrow morning and would be ready to cement the casing around 9:00am to 10:00am tomorrow morning.
1345506119 2012-08-16 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 13 3/8 surface casing. My inspection revealed that Patterson Rig # 261 has drilled a 17.5 hole to a depth of 548. Wyoming casing ran 548 of 13.375 casing into the well with six centralizers installed on the casing string (on joints 1,2,3,6,9,12). O-Tex cemented the casing with 510 sacks of Class H cement (2% CaCl; .27% Cello-Flake; and .4% C-41P) with a density of 15.6 pounds per gallon and a yield of 1.2 cubic feet per sack. There was good circulation throughout the job and approximately 20 barrels of cement was circulated to surface. My inspection revealed no violations at this time.
1450285248 2017-09-14 Production Wells No Violations I conducted a status check of the well and production pad. The well is in production and no changes have been made to the well or pad since my last inspection. I performed a walk-around inspection of the site and did not locate any indications of sedimentation or erosional issues. No violations were noted.
1499292906 2017-06-29 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The #6H well is in production and consists of a plunger-lift assembly. Jersey barriers were installed around the wellhead. The well is identified with a legibly-marked well identification sign. Identification was posted at the tank battery which consisted of ten 400 bbl production tanks, four labeled as containing produced water, and six labelled as containing condensate, and one low-pressure. The tank battery is surrounded by a dike. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Production equipment on the well pad consisted of:

5-producing wells 10- 400 bbl production tanks, four labelled as containing produced water, and six labelled as containing condensate 5 sand separators 5 gas processing units 5 low pressure vertical separator 5 vapor recovery units 1 horizontal separator 1 compressor station 2 oil stabilization units. Jersey barriers are installed around the production equipment. The access road and man doors were locked. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road was stable in in good repair.

1783596412 2012-08-19 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 9 5/8 casing. My inspection revealed that Patterson Rig # 261 has drilled a 12.25 hole to a depth of 2100. Wyoming casing ran 2093 of casing into the well bore with 19 centralizers installed on the casing string (on joints 1,2(x2),3,6,9,12,15,18,21,24,27,30,33, 36,39,42,45,48). O-TEX cemented the casing with 790 sacks of ClassH cement (10% salt - expansion agent; .27% Cello-Flake; .4% C-41P (powder defoamer); .1% C35 (dispersant). There was good circulation throughout the job and 30 barrels of cement was circulated to surface. My inspection revealed no violations at this time.
