0143258314 2013-03-21 Drill / Deepen / Reopen No Violations I was on site to check the status of Nomac Rig # 311. My inspection revealed that Nomac 311 has rigged up and are ready to spud. I talked with Wade (rig consultant) and he advised that they should be ready to spud anytime and should be finished drilling by early tomorrow morning. He also advised that they had to log the 17.5 hole, since this was the first well on the pad. My inspection of the well site revealed no violations at this time.
0557941481 2015-09-28 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is equipped with a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly marked well identification sign. The tank battery consisted of six 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock is installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

0620801175 2015-01-20 Production Wells No Violations I was on location to conduct a status check of this well. The well is of plunger lift construction. Jersey barriers were installed around the wellhead. The tank battery consisted of 6, 400 BBL production tanks and was diked. A windsock was installed on the top of the production tanks. 1331741907 2017-12-12 Production Wells No Violations I was on location to conduct a status check of this well. The pad is equipped with one producing well, and the well was equipped with a plunger lift assembly. Proper identification was posted at the access road entrance, the pad entrance, the wells, and tank battery. The tank battery was surrounded by a steel dike. Other production equipment on the well pad consisted of:



The lease road was in good repair. No violations were observed at the time of inspection.

Originally Chesapeake had advised that Nomac Rig # 41 was going to move onto this site, but Nomac Rig # 41 has went to work for Range Resources instead of drilling this well. My inspection revealed no violations at this time.
2045432184 2012-06-26 Drill / Deepen / Reopen No Violations I was on site to check the status of the well site construction. My inspection revealed that the lease road and the well site has been constructed. All the erosion and sedimentation controls have been installed and are functional.
