0274838426 2013-01-30 Completion Testing No Violations During the cementing of the Booth 3H 5.5 production casing I noticed flaring gas in the distance that appeared to be coming from a Nomac drilling rig. I drove to the location and found out that excess gas was being flared from this location with a completed well, not the drilling rig. I was informed that they will spend the next several days bleeding off excess gas and that flaring would continue.
0437536517 2015-12-30 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:


Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1336508739 2012-05-07 Drill / Deepen / Reopen No Violations ODNR Inspector Richard Anderson and I arrived on site to check the status of Patterson Drilling Rig # 329. The rig is running the drill string and the stinger back into the hole at this time. They are going to set the kick plug sometime this morning. The drilling consultant advised that they should be ready to start pumping the kick plug around 11:30 AM today. We advised that we would return to the site to witness the plug back. We departed the site and went to the Coe site in Carroll County. At approximately 11:28am I called the drilling rig, via my cell phone, and they advised that Schlumberger Cementing was running behind and the plug back should take place later this evening. At approximately 7:32 PM, I received a phone call from the drilling consultant and he advised that they should be ready to start cementing around 8:30 PM.
1337341135 2012-05-16 Drill / Deepen / Reopen No Violations Division of Oil and Gas Resources Management Inspector Rick Anderson and I were on site to check the status of Patterson Drilling Rig # 329. Our inspection revealed that the drilling rig had reached a total depth of 14345 and have most the drill string tripped out of the hole (standing in the derrick). The rig is coming out of the hole with the directional tools and laying them down. Chesapeakes Drilling Consultant , Danny Lambert, advised that they should be ready to start running the 5 1/2 production casing within the next hour. Our inspection revealed no violations at this time.
