0767590871 2018-10-29 Production Wells No Violations The well was equipped with a plunger-lift assembly. The surface-intermediate annular gauge displayed a reading of zero psi. I performed a walk-around inspection of the pad and no violations were observed at the time of inspection.
0801368524 2013-01-03 Drill / Deepen / Reopen No Violations I inspected this drilling operation on 1/3/2013. UDI Rig # 63 was on the location but not drilling due the Top Drive being down and waiting for repair. UDI spudded 1/3/2013 and drilled to a depth of 191 TVD before incurring the breakdown. All activities observed were in compliance.
1412691549 2014-05-01 Production Wells No Violations I was on location to investigate complaint #4690 concerning possible vibrations caused by oil and gas activities. This pad is located 1 mile NW of the Complainants property. My inspection found that the Graham #s 1H & 2H (PN# 22244) are currently not in production. Rex Energy personnel were on location with a coiled tubing unit preparing to set bridge (kill) plugs in both wells in order to re-complete the #2H. They will begin re-completion operations starting on 5/12/2014. I found no source for vibrations on this pad. No violations.
1518290892 2017-02-01 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is equipped with a plunger-lift assembly. Jersey barriers were installed around the wellhead. The well is identified with a legibly-marked well identification sign. Identification was posted at the tank battery which consisted of eight 400 bbl production tanks, four labeled as containing produced water, and four labelled as containing condensate. The tank battery is surrounded by a dike and no standing water was present inside of the tank battery at the time of inspection. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Production equipment on the well pad consisted of:

2 producing wells 8- 400 bbl production tanks, four labelled as containing fresh water, and four labelled as containing condensate 2 sand separator 2 gas processing units 1 vapor recovery unit Jersey barriers are installed around the production equipment. The access road was locked Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The lease road was stable in in good repair.

1532375143 2018-06-27 Production Wells No Violations I conducted a status check of the well and production pad. Two wells are located on the pad (permit numbers 22133 & 22244). This well is equipped with a plunger-lift assembly. The surface--around inspection of the pad perimeter, and erosional controls appeared to be functioning.

Equipment on the pad consisted of the following: Eight 400 bbl production tanks; four labelled as containing condensate and four as containing produced water Two gas processing units Two vertical separators One combustor unit
