0124237495 2014-04-21 Drill / Deepen / Reopen No Violations I was on location to inspect drilling operations. Nomac Rig #73 was drilling the 8.75 section of the well at a depth of 4,369. The mud-up-point will be 6,839 and they will begin drilling the curve at a depth of 6,939. The location is in good condition and all equipment is on containment. No violations.
0668913773 2014-04-18 Drill / Deepen / Reopen No Violations I was on location at 10:00am to check the status of drilling operations. Nomac #73 has drilled the 17.5 surface hole to a total depth of 755 and is currently circulating to condition the hole. I inspected the 13.375 surface casing and found it is new, 54.5lb/ft., J-55 casing. I inspected the float equipment a found a new float shoe and float collar manufactured by Weatherford. There are seven new, bow-spring centralizers in front of the casing rack. At 10:56am, Nomac began to trip drill pipe out of the hole and I left location. No violations.

I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The #5H well is in production and is equipped with a plunger-lift assembly. The production tanks are surrounded by a steel dike. The equipment on the pad consists of the following: 6 producing wells

1 Low pressure vertical separator 1 heat treater 6 sand separators 6 gas processing units 1 combustor unit


1697207509 2015-12-04 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is equipped with a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1883324515 2014-04-18 Casing String Witnessed No Violations I was back on location at 1:45pm to witness the cementing of the 13.375 surface casing. Wyoming casing ran pipe to a depth of 745 and O-Tex rigged up their cement head. The casing was cemented with 147 barrels of cement weighing 15.6lbs/gal on the pressurized scales. 39 barrels of cement circulated to surface. See casing ticket for more details on this job. No violations.
