0348304265 2012-07-31 Production Wells No Violations I received a call at my residence from the Carroll County Sheriffs Department at approximately 6:57pm advising me that there was an oil/gas well spewing oil and gas into the air. Jimmy (Carroll County Sheriffs Department) advised that the well was located on the west side of Azalea Road near the Carroll / Tuscarawas County line. He also advised that Leesville Volunteer Fire Department was on site and that he was going to contact Tom Cottis (Carroll County EMA). I advised that I would be enroute to the site immediately. I departed my residence and arrived on the scene at approximately 7:25pm. When I arrived on the scene, I observed the Leesville Volunteer Fire Department and Tom Cottis (EMA) were on site. They advised me that they had several calls of a very strong gas odor in this area since yesterday and somebody finally found the source today. My inspection revealed that the gas leak was approximately 50 yards west of the tank battery. Petrox Inc and S&S Energy own wells that are utilizing the common tank battery. S&S Energy owns the Clark-Morrison #8 well (permit number 1293) and Petrox Inc owns the E. Smith #2 and #3 wells (permit numbers 1283 and 1305). I called Jason Scott and Brian Coblentz (both with S&S Energy) and left messages on their voice mail to call me immediately. I also left a message on Brian Davies cell phone (contract pumper for Petrox) and advised him to call me immedately. I received a call from Brian Coblentz and Brian Davies and advised them of the situation. They advised me that Brian Davies would respond to the situation because he was located approximately 25 minutes away versus Brian Coblentz being approximately 1.5 hours away from the site. Brian Davies arrived on site and we walked the line and I pointed out the leak. We walked back to the tank battery and he shut the valve on the gas sales line to isolate the line. Brian Davies dug up the line with a shovel and put a line clamp on the line to fix the leak until more permanent repairs
