1113545102 2016-02-03 Production Wells No Violations I was on location to follow-up on an oil release that occurred on 1/31/16 and check the condition of the containment measures following heavy precipatation last evening. I met with the landowner, Mr. Pat Weaver, who was allowing Petrox and the contractors to access the location through his property. There were no personnel on location to monitor, assess, or conduct remedial operations during my inspection. I walked the entire course of the creek up to the point source of the oil release. I had to adjust several booms along the lower course because the stream had widened several feet due to the precipatation event. The four siphon dams were functioning properly and containing oil on the upstream side. The lowest of the siphon dams was beginning to leak water in between the siphon pipes. I inspected the pond and outflow into the creek and found no evidence of oil or sheen. A boom placed downstream of the outflow was clean and functioning properly. Inpector Ashley Raff, OEPA, and DOGRM Emergency Reponse Coordinators will continue to monitor this location.
1460047829 2016-02-02 Production Wells No Violations I was on location with Emergency Operations Coordinator Vikki Bunting to follow up on an oil release that occurred on 1/31/16. Banjo Contracting, Petrox, and Scott Miller Contracting were on location to continue with remedial operations. A fourth siphon dam was installed past the lowest dam in preparation for heavy, forcasted precipatation overnight. Scott Miller rebuilt the upper siphon dam with two (2) corrugated, 8 pipes to handle a larger flow of water. I asked Scott to attempt to clear access to the creek bed below the first siphon dam to allow access to pockets of oil collected along the course. We walked downstream to the pond and all containment measures were functioning properly. I will continue to monitor the location.
2044378522 2016-02-01 Production Wells No Violations I received a call from Carroll County DOGRM Inspector Ashley Raff at 5:27pm to notify me of a crude oil release at this location and asked me to respond Monday morning 2/1/2016. I was on location this morning with Kurt Kollar (OEPA), Tom Kottis (Carroll EMA), Petrox, and Banjo Excavating to assess the release. The release occurred at a riser pipe where this well and the #3A (PN#21254) tie into the flow line to the tank battery. According to Petrox, 1.25 flow line had been recently inserted in the old 2 steel line and when the well was turned on, the flow line ruptured at a connection at the riser. They estimate two (2) barrels of oil was released. The oil has impacted the ground surface in an area of 10 X 40 to the south of the riser. The product entered an unnamed tributary of Conotton Creek and travelled southeast approximately 0.25 miles and was contained prior to entering the landowners pond. Three siphon dams were installed along the course of the creek to contain the product. The affected ground surface had been covered in Peatsorb. I walked the creek, which was laden with brush and debris, and there were heavy pockets of trapped oil along the upper course between the siphon dams. Scott Miller Contracting arrived on location and began excavating the contaminated soil (source) and moving it to a diked and lined containment area near the lease road. Banjo was collecting saturated pads from behind the dams and began flushing the creek with a 3 trash pump and hose between the first two siphon dams. I will continue to monitor clean-up progress at this location.
