1323965277 2011-12-13 Production Wells Violations Noted At 5:57 PM on 12-13-2011, I received a phone call from Mark Rogers of the ODNR Communications Center (614-799-9558) about a crude oil leak at the referenced site. This complaint was forwarded to him from the Ohio EPA, which was received on there hotline at 5:30 PM on 12-13-2011, from Steven Friedman (330-866-9177). While enroute to the site, I was contacted by Wade Balser, the On Scene Coordinator of the Ohio EPA (216-789-9281) about the reported release at this site and his estimated time of arrival . I contacted Mr. Friedman to ask if he could show me the actual location of the release. He said he could not but he would mark the location. I contacted and/ or left a messages with the following: Gene Chini, North Regional Supervisor ODNR (330-284-2942, Tim McConaha (330-602-5551) and Charlie Youngen (330-575-9262) of Enervest. I arrived at the site marked by Mr. Friedman at approximately 6:50 PM and found crude oil in the drainage ditch on the east side of Swift Road . The ditch was filled with vegetation which impeaded the fluid flow in the ditch which is to the south. The gradient of the property adjacent to the source point is to the west. I placed two (2) sorbent booms in the drainage ditch, one approximately 25 south of the location and the second at the end of visible impact. ODNR Supervisor Gene Chini arrived on location at approximately 8:30 PM and we searched the field adjacent to the ditch for the source of the release. Approximately 30 directly upgradient from the point of entry of crude oil into the ditch was a portion of exposed flowline that had been patched. It was apparent that this was the cause of the release. It was not apparent if the flowline was still leaking. I contacted Wade Balser (Ohio EPA) and notified him of the situation. Charlie Youngen (Enervest) called and said he would contact the appropriate personnel to initiate the containment and clean-up of the release. We continued oil clean-up with sorbent pads u
1324495429 2011-12-20 Production Wells No Violations Followup inspection revealed siphon dam had been removed and straw bales were placed where siphon dam had been. 2 sorbent booms and a plywood dam were were still in place in the ditch. I contacted Charlie Youngen, the Enervest Productin Manager, on 12-21-2011 and he advised me the Wade Balser of the Ohio EPA approved the removal of the siphon dam and witnessed two (2) loads of contaminated soil being removed from the release site. I checked with Inspector Balser on 12-21-2011 and he confirmed clean-up progress at the site. A visual check showed that the well is still shut in.
1337262235 2012-05-17 Production Wells No Violations My inspection found the Notice of Violation (NOV) requirements have been met. The well is producing and no violations are noted at this time.
1458223244 2016-03-16 Production Wells No Violations I was on site to follow up on the cleanup of the oil spill reported at on 10:15 PM on 03/16/2015. The oil had been completely contained by the operator, and booms and diking material were deployed on 03/16/2016. Present at the location were Tom Vohsik and Steve Spellmen of Enervest Operating, Reginald Brown of the Ohio Environmental Protection Agency (OEPA), and Tom Cottis of Carroll County Emergency Management Agency (EMA). The spill originated from a flowline break, and the coordinates at which the break occurred are (+-9) 40.69959, -81.23688. The estimated volume of oil released is less than one bbl. The escaped oil flowed south, approximately 215 feet along a roadside ditch on the east side of Swift Rd, where it then flowed through a road culvert and crossed onto the west side of Swift Rd. Oil sheen and minor amounts of free oil were visible along the ditch. The flow apexes into a retention pond in an AEP landfill property in Stark County. The retention pond appeared to contain amounts of oil contamination, but further inspection found that the residue was likely biologic in origin and not related to the oil release. Spill cleanup was ongoing, and two siphon dams were in the process of being installed during my inspection.
1492006829 2017-04-23 Production Wells No Violations I conducted a status check of the well. The well is equipped with an electrically-driven pumpjack and the unit was producing at the time of inspection. The tank battery is labelled as sharing production equipment with PN# 20707 and consist of one 210 bbl production tank, two vertical separators, and one horizontal separator. The tank battery was surrounded by a four feet high fence which was partially down. A meter run with a current chart labeled for PN# 21104 was located outside of the dike.
