0629736324 2017-06-05 Final Restoration No Violations I conducted a final restoration. The well was plugged on 11/07/2012 and a plugging permit is on file. Production equipment has been removed but final restoration cannot be passed due to tubing and power line poles located around original location of the wellhead. The flowline was partially buried and exposed near the location of the well head. I contact Duane Clark from Petrox and requested either a landowner waiver, or removal scrap metal and wood, and the flowline to be removed before final restoration can be passed.
0705284240 2012-11-06 Plug / Plug Back No Violations I was on site to witness the pumping of the Clinton plug. They mixed 25 sacks of Class A cement (15.6 density and 1.18 yield) and pumped the plug thru 2 3/8 upset tubing. They displaced the plug with 20 barrels of fresh water. The plug was from 5450 to 5182. My inspection also revealed that the plugging pit was lined and was of sufficient size to contain all the water circulated back out of the hole during the plugging process. My inspection revealed no violations at this time.
1337343320 2012-05-15 Production Wells Violations Noted My inspection found the well idle and the record shows the last production reported was for 305 barrels of brine in 2009. Plug, produce or sell this well. Failure to comply with this request may result in forfeiture of bond.
1352225163 2012-11-06 Plug / Plug Back No Violations I was on site to check the status of the plugging operations. My inspection revealed that Nicks Well Plugging service rig was getting ready to start pulling the rods. Terry Jones (service rig operator) advised that they should be ready to pump the Clinton plug thru the tubing around 12:00 (noon) today. My inspection also revealed that they havent excavated the plugging pit at this time. I advised that I would return to the site later to witness the Clinton plug.
1499694077 2017-07-10 Final Restoration No Violations All debris and electric poles at the well location have been removed. The remaining buried flowline has been removed as verified by Petrox. A landowner waiver for the lease road was received by the Division on 07/05/2017. Final restoration can be passed.
