0184620718 2018-05-29 Production Wells No Violations I was on site to conduct a compliance notice follow-up from a release which was discovered on 05/08/2018. The diked area around the well head contained water with oil product, and the water appeared to have breached the contained area. Sheen was present in the excavated area around the well. I met with the George Miller of William S Miller Inc. on site who agreed to plug the well during June of 2018. I stated that any product needs to be removed and recommended in the interim of plugging the well that a lined pit be installed near the well to contain any further contamination leaching from around the well, and the pit be regularly monitored for water levels. The compliance notice will be closed once all visible contamination has been removed.

0348137348 2018-05-15 Production Wells No Violations I conducted a compliance notice follow-up on the well for pollution and contamination. The area around the well head, which was excavated and diked, contained water and visible oil product. Oil soak pads were in place. The well is located a low section of a farm field and intersects a natural drainage path. A shallow ditch west of the well was put in place to catch any drainage, and a boom was deployed. The boom appeared clean at the time of inspection. Cleanup was on going at the time of inspection. A stockpile of excavated and contaminated soil was located north of the well and on a liner proper disposal.
0803697649 2015-05-20 Production Wells Violations Noted Inspection showed the well is equipped with an electrically powered pump jack. The tubing valve is open. There is a line with a closed valve that is partially buried that may be the flow line or an old domestic line. The well head is overgrown with vegetation. One, 100 bbl. White rusty steel tank and 1, 50 bbl. Green steel tank are near the well. There is not a meter or separator at the tanks. The identification at the well shows G & O as the owner. The Division of Oil and Gas records show William Miller is the owner. The tank battery is overgrown with vegetation. There has been 0 production reported from 2012 through 2014. 0 barrels of oil were produced between 2005 and 2007. 18 barrels of oil were produced in 2008.0 barrels in 2009 and 11 barrels of oil in 2010. This well used to produce into the same tank battery as P. 435, 437 and 438. All four wells reported the same production. G & O and William Miller have been contacted to determine ownership and plans for this well.
0860511096 2018-12-11 Final Restoration No Violations I was on site for a final restoration inspection. The equipment and tank has been removed, but a pile of scrap was still present on site. It also appeared that the area still needed more grading and seeding. I will contact the operator. I witnessed plugging operations for this well which were finished on 07/07/2018. Plugging of the well has been fulfilled and the compliance notice issued to plug the well can be closed at this time.

1135914317 2018-07-08 Plug / Plug Back No Violations I was on site to verify the cement top after plugging operations commenced on 07/07/2018. The service rig was off location and the temporary pit which was used during plugging operations had been emptied. I located the cement top to be at surface.
1275162986 2013-08-22 Administrative Inspection No Violations I was on inspection with Helen Halter. My inspection found the well to be shut in. Last reported production was in 2010, the operator reported 11 barrels of oil produced. There was no identification posted at the well or the tank battery. The well needs put back into production or plugged. Identification needs posted at the well site. 1353779248 2018-05-21 Production Wells No Violations I conducted a compliance notice follow-up inspection of the well for a release which was discovered on 05/08/2018. The water around the well head was near the brim of the dike and contaminated oil pads as well as free product was visible on the water. I contacted David Miller of William S Miller Inc. and requested the contaminated water be emptied to prevent overflowing. He stated a pumper was in route to change the contaminated oil soak pads. We also discussed the plugging schedule of the well which he stated should take place in June of 2018.
1357819755 2018-05-31 Production Wells No Violations I was on site for a compliance notice follow-up for the well causing pollution and contamination. A lined pit had been installed south of the well head to contain any further contamination leaching from the soil, and will remain in place until plugging operations commence. The pit was dry at the time of inspection. The contamination in the area west of the well head, which was visible on 05/29/2018, was no longer present, as well as most of any product located in the diked area around the well. The compliance notice will remain open at this time until further monitoring to ensure that all pollution and contamination has been removed. The precautionary containment pit is planned to stay in place until plugging operations are finished.
1446047373 2015-10-26 Production Wells No Violations Inspection showed the well is not in production and there is not a domestic connection. Weeds have been cut down around the tank battery and pump jack. A new Identification sign with William Miller, Inc. as owner has been posted. Bill Miller Jr. has been in discussion with Supervisor Hill and would like to put together a timeframe for plugging or producing the wells they own in this area that are not in production. The current compliance date for plugging this well is 12/01/2015.
1458764444 2015-10-15 Production Wells No Violations I conducted a status check of this well with DOGRM Regional Supervisor Tom Hill. The well is equipped with an electrically-driven pumpjack. The production valves were in the open position, but the polish rod did not appear to be lubricated, and the joints of the pumpjack showed signs of corrosion. The tank battery consists of one 100 bbl production tank and one 50 bbl brine tank. Proper identification had been posted at the well. A review of records in the State database shows that the last reported production was 11 bbls in 2006. The well is not equipped to produce at this time. The operator shall submit a plugging schedule to inspector Ashley Raff for approval within 30 days of 11/06/2015 at: Ohio Department of Natural Resources, Division of Oil and Gas, 3575 Forest Lake Drive Suite 150, Uniontown, Ohio 44685, per OAC 1501:9-11-05(B). The well owner/operator will be notified if the plugging schedule is approved or denied. As per the Compliance Notice on 06/01/2015, the well is to be plugged by 12/01/2015.
1480582169 2018-12-19 Production Wells No Violations I spoke to David Miller of William S. Miller on 12/13/2018 and informed him that I could not pass final restoration during my last inspection on 12/11/2018 due to debris from the well present on site and because the area did not appear graded and seeded. He stated that he was unaware that any debris was on site. As of my inspection on 12/19/2018, the stockpiled debris was still on site and further grading needed to be done. In response to the compliance notice issued on 05/18/2018 for the well causing pollution and contamination, all contamination appeared to have been remediated so the compliance notice can be closed.
1527775855 2018-05-08 Production Wells Violations Noted I conducted a compliance notice follow-up which was issued to plug the well on 01/06/2015. At the time of inspection, a leak appearing to come from around the well head was active, and contamination and mobile oil was present measuring approximately 50 x 10. The contamination travelled west in the field for approximately 150 west where visible contamination and scald diminished. I contact Dave Miller of William S Miller Inc, who met me on site. Visible product was excavated and area of well head exposed where it was discovered that the leak appeared be from a failed section of flowline near the well. An area around the well head was dug out and diked to holed water and further contamination leached from the soil. Clean up is ongoing. The operator agreed to apply for a plugging permit and plug the well as soon as possible.

-1000 lbs. The plug was set from 5492-4980 with 50 sacks of Class A cement with cottonseed hulls. I weighed the cement at 15.4 ppg. The pH of the mix water was 7.0 and chlorides were <-in overnight.

1938592777 2018-10-30 Final Restoration No Violations I was on site for final restoration. The production tank had been removed from site, but debris from the well was still present on location and vegetation had not yet been established. Final restoration cannot be passed at this time.
