0040185183 2019-03-18 Production Wells No Violations I was on site for a compliance-notice follow-up after being requested by the landowner because of a down power line. A power line running to the well was down on the ground. I was informed there was no electricity to the line. I contacted the operator to have this repaired as soon as possible. The production valves to the well were in the closed position. The compliance notice remains open. I will contact the operator for an update on their plugging plan.
0071888443 2015-03-20 Production Wells No Violations I was onsite responding to complaints of an oil spill (complaint #4970). I received a call at 4:07PM on 3/20/2015 from Regional Supervisor Tom Hill asking me to investigate a report of oil leaking from the tank battery of the Rinehart JD #1 (34-019-20431-0000) on Stallion Road in Carroll County. I arrived onsite at 5:05 PM followed by Dave Widuck, a contractor for G&O Resources, and ODNR DOGRM inspector Heidi Scott. At the tank battery, the release had already stopped prior to my arrival. The base of the 210 bbl storage tank failed due to corrosion and released its contents into the dike, estimated between 10-
0268565029 2015-03-23 Production Wells No Violations I was on location to follow-up on an oil spill initially investigated by Inspector Josh Schiering over the past weekend. I was on location with Inspector Romeo, Kurt Kollar (OEPA), Tom Kottis (Carroll EMA), and Scott Miller Contracting. The contractor has scraped and stockpiled most of the affected soil from the drainage ditch and has it covered with a tarp to prevent leaching. We walked the drainage ditch down to the pond where a hard boom had been placed to collect any residual oil from the ditch and a contractor was soaking the oil up with pads. We inspected the pond and it was relatively free of oil except for small blotches on the north side of the pond. A plywood underdrain dam has been constructed around the pond overflow to prevent any oil from migrating out of the pond and into the drainage creek. The creek was not impacted by the spill. We inspected the tank battery and determined that the 210 barrel tank had rotted out at the base of the clean-out hatch and the oil flowed through a cut in the dike and to the ditch at Stallion Road. A large, fire-heated, water knock-out was located at the rear of the tank battery and was no longer in use. The base of the knock-out was badly rusted and poses a safety hazard. The battery also contained a horizontal separator and a chart box on the sales line. The valves on the separator and sales line were closed, but gas was bubbling to the surface where the sales line entered the ground and indicates that the sales line is leaking and in need of repair. The ID posted on the tank is inadequate and indicates the Rinehart #s 2 & 3 (it should indicate #s 1 & 3) with no permit numbers on the ID tag. The contaminated soil at the tank battery has been partially excavated and covered until it can be disposed of properly. Bill Lavin (G & O) arrived on location and spoke with Kurt Kollar. I spoke with Bill about the issues at the tank battery and he is going to have Scott Miller take down the water knock-out and dispo
0355953733 2015-04-15 Production Wells No Violations I was on location to follow-up on an oil release at this tank battery. Scott Miller has addressed the issues that occurred on 4/9/15 and bermed the contaminated soil pile. He installed a 12 siphon dam at the berm and it is functioning properly and containing the oil. The bad 210 barrel tank has been pulled from the battery and the large heat treater has been laid on its side. This equipment will be removed after May 1st when the frost laws are lifted. This clean-up continues.
0919098014 2015-04-09 Production Wells No Violations

In response to Complaint ID #4970, DOGRM Inspector Mike Dinsmore and I went to the location of the tank battery. Heavy rainfall had caused the previous spill remediation efforts to fail. Oil had leached from the dike that was constructed adjacent to the tank battery. Oil sheen was present along the ground into the drainage ditch adjacent to the battery and on the road. The siphon dam that was placed in the stream on the north side of the road was not functioning and broken down. I walked the course of the stream on the property. The stream was clear, but there were some pockets of sheen in the floodplain next adjacent to the stream. The underflow siphon dam that was built in the pond was functioning properly, and outflow from the dam was clear. Scott Miller from G & O arrived on site at 8:00 pm for clean-up efforts. At the time we left they were to clean up any escaped oil residue, and build up a siphon dam adjacent to the tank to contain any oil.

1427294896 2015-03-22 Production Wells No Violations On site to monitor the remediation of the oil spill that occurred on Friday, 03/20/2015, when the production tank on the south side of Stallion leaked oil through a rusted heater plate. The oil exited over the northwest corner of the dike and entered the road ditch that is running water. The road ditch drains into a road culvert that crossed N. under Stallion Rd. into a drainage ditch that runs through Scharvers pasture into Scharvers pond. Scott Miller Contracting is on site remediating the oil spill. Oil contaminated soil from the tank battery is being stockpiled at the tank battery for removal. A small track hoe, skidsteer and tractor are removing oil contaminated soil and erosion material (bricks, rocks) from the drainage ditch. Approximately 1/2 of the drainage ditch has been remediated. An electric fence has been installed to isolate the cows from the drainage ditch. The oil contaminated material is being stockpiled in a bermed area in the pasture by the road and will be covered with plastic prior to removal. Siphon dams are installed in the road ditch on the south side of Stallion Rd. and at the north side of the road culvert. Inspection of the pond showed oil booms are present at the south and north ends of the pond. Oil collecting at these points is being removed. Boards and a boom are placed around the spillway to prevent oil from exiting the spillway. A light oil sheen and strings of oil are present on the west side of the pond. Wind direction is to the west. A boom is present in the drain way approximately 25 downstream from the spillway pipe. A diaper with minimal oil staining is present on the upstream side of the boom. There is no evidence of oil downstream from the boom in the drain way that feeds into a pond on an adjoining property and Huff Run during todays inspection.

1432153611 2015-05-19 Production Wells No Violations On site to monitor the on site remediation of the oil contaminated soil that is stockpiled on the south side of Stallion Road on the Scharver property from a spill that occurred on 03/20/2015. The soil was stockpiled west of the gate that is west of the machinery shed near the road. Don with Lakeside Excavating is mixing the soil with oil free soil and manure and will add lime when mixing is complete. The majority of the stockpiled soil was not oil contaminated but was removed with the oil contaminated soil during the initial clean up on the south side of Stallion. Remediation will continue tomorrow, 05/20/2015. Inspector, Ashley Raff and District Supervisor, Rick Anderson will coordinate further coverage of the site remediation.
1439472042 2015-08-05 Production Wells No Violations I conducted a compliance notice follow-up on this well with DOGRM District Supervisor Rick Anderson. The compliance notice stemmed from an oil-spill that occurred on March 22, 2015. The stockpiled oil-contaminated soil at the tank battery on the south side of Stallion Road has been stabilized and removed. Any remaining stabilized soil has been seeded, and the soil was vegetated. The contaminated soil on the north side of the Stallion road has also been stabilized or removed. The siphon dam and booms in the stream and ponds have been removed, and no oil was present in or around the water. The corroded heater treater separator, along with the two production tanks, two-100 bbl brine tanks, and the meter run at the tank battery have been removed. The violations have been resolved and the compliance notice can be closed.
1441291853 2015-09-02 Production Wells Violations Noted I was on site to conduct a status check. The well is equipped with an electrically-driven pumpjack. There was a fence surrounding the well which was open. There was identification posted containing the emergency contact phone number for the operator, but the identification contained no permit number for the well. The tank battery is dismantled due to an oil spill that occurred on March 22, 2015. A review of records in the State database shows that the last reported production was 4 mcf of gas in 2009. The well is incapable of production due to the absence of any production equipment at the tank battery. The operator shall submit a plugging plan to inspector Ashley Raff for approval within 30 days at: Ohio Department of Natural Resources, Division of Oil and Gas, 3575 Forest Lake Drive Suite 150, Uniontown, Ohio 44685.
1491364870 2018-09-20 Production Wells No Violations A compliance notice was issued for this well in 2015 for non-production and lack of identification. Identification was posted at the well head. The tank battery equipment was removed in 2015, and has not yet been replaced. No production has been reported since 2008. The production valves appeared to be in the closed position. There was a pool of standing water and agricultural run-off around the well head. I did not observe any sheen/ oil in the standing water. I tested the standing water for chlorides using a low-range Quantab which concluded in a reading of 290 ppm. I will be meeting the operator to discuss a plugging schedule.
1510155806 2017-11-02 Production Wells No Violations I conducted a compliance-notice follow-up on the well. Reported production for the well has been zero since 2010. The well is equipped with an electrically-driven pumping unit, and identification was posted at the well. I was unable to determine with certainty if the production valves were in the open or closed position. All production equipment at the tank battery has been removed, and I did not locate a sales meter. I contacted G&O Resources on 11/08/2017 and requested a call-back to discuss plugging the well.
1949589224 2019-03-18 Production Wells No Violations I was on site after receiving a call from the landowner stating an electric line to the well was down. I contacted the contractor for the operator who stated the line is not live, and that an electrician has been contacted repair the line as soon as possible. The well was equipped with a pumping unit and does not appear to be currently producing. I will be contacting the operator regarding the plugging status of the well.
1983964777 2015-07-14 Production Wells No Violations I conducted a follow-up to a compliance notice stemming from the spill that occurred on 03/21/2015. The production equipment has been removed from the tank battery. All of the contaminated soil at the tank battery has been excavated out and disposed of. The area has been seeded, and vegetation is growing. No contaminated soil was visible on the property north of the road. I walked the length of the stream and around the pond. No oil-sheen or contaminated soil was visible in or around the stream. No contamination was visible in the stream. All of the booms and siphon dams have been removed. The compliance notice can be closed.
2020709513 2015-04-09 Production Wells No Violations I was on location with Inspector Ashley Raff in response to a call from Rose Township about failed containment measures put in place to control an oil spill. My inspection found that the heavy rains today had overwhelmed a small siphon dam on the south side of Stallion Road and had allowed oil leaching from a covered stockpile in the tank battery to go around the dam and put a sheen on the road. A 12 inch siphon dam on north side of the road was completely washed out and allowing the leached oil to migrate down the drainage gully. A hard boom at the mouth of the drainage channel has prevented the oil from reaching a pond. I called Scott Miller Contracting, who installed the measures, and talked with Scott about the situation. Scott arrived on location after 45 minutes and began addressing the issues. He was going to build a berm around the contaminated soil pile that was leaching out and install the 12 siphon dam right at the source to prevent further run-off of oil. I will continue to monitor the location until the clean-up is completed.

The well is equipped with an electrically-powered pumpjack. The well shares a tank battery with the Rinehart #3 well. The tank battery has been dismantled due to an oil spill that occurred on March 22, 2015. The fence surrounding the well was open, as was the electric control panel to the pumpjack. A review of records in the State Database shows that the last reported production was 4 mcf of gas in 2009. I will contact the operator regarding installing proper identification, and the production status of this well.
