0701452622 2019-05-05 Casing String Witnessed No Violations Precision Rathole drilled a 30-inch diameter borehole to 100 feet in depth and installed 100 feet of 24-inch diameter conductor casing with welded joints. The conductor weighed 94.62 pounds per foot. Water was encountered at 90-feet during the drilling process. During my inspection a pre-job safety meeting was held with all personnel on location and Petroset cementing rigged up and pumped 220 sacks of class A cement with 2 percent calcium chloride and displaced with 47 barrels of fresh water. Cement returns were established and were used to cement the cellar floor to create a liquid-tight cellar as per Ohio Administrative Code 1501:9-1-08 (F).
1557756924 2019-05-10 Casing String Witnessed No Violations The purpose of my inspection was to witness a mine string cement job. Patterson 577 along with the assistance of Wyoming Casing Crew set the 18.625-inch diameter casing at 365 feet from the rig floor. Patterson 577 rig floor is at 25 feet from ground level. A mine void was encountered during the drilling process from 308-feet to 315-feet. A cement basket was set at 217. C&J Energy Services plumbed up to the 18.625-inch casing pressure tested their lines and pumped 127 sacks of cement. A cement sample was pulled and weighed in pressurized mud scales confirming the weight of 15.6 pounds per gallon. The plug was dropped and displaced with 104 barrels of displacement. After landing the plug the pressure was increased 500 pounds per square inch and held for 5 minutes. The floats held and .5 barrel was bled back to the truck. 1-inch diameter steel tubing was ran to 217 feet and C&J topped off the 22-inch borehole by 18.625-inch casing annulus with 201 sacks of cement.
1557838740 2019-05-13 Casing String Witnessed No Violations The purpose of my inspection was to witness the cementing of the 13.375-inch diameter surface casing within the 17.5-inch diameter borehole. Patterson 577 along with the assistance of Wyoming Casing Crew set the casing at 1615 feet from the rig floor. Patterson 577 rig floor is at 25 feet from ground level. During my inspection a safety meeting was held with all personnel on location. Following the safety meeting C&J Energy Services pumped 35 barrels of gel and 5 barrels of fresh water ahead of 1170 sacks of class A cement. The cement was weighed in pressurized mud scales confirming the weight of 15.6 pounds per gallon. The plug was dropped, and 237 barrels were pumped for displacement. Estimated 13 barrels of cement returns established. After landing the plug the pressure was increased 500 pounds per square inch and held for 5 minutes. The floats held and .5 barrel was bled back to the truck.
1715935787 2019-05-24 Casing String Witnessed No Violations The purpose of my inspection was a status check. During my inspection Patterson 577 along with the assistance of Wyoming Casing Crew were running 23 pound per foot, P-110, 5.5-inch diameter production casing. Installation of the casing centralizers were observed and appeared to meet the requirements within the Ohio Administrative Code chapter 1501:9-1-08 (K). The Ascent Company man on location stated that the planned set depth was 18379 feet. The cement job proposal was reviewed and discussed. 3365 sacks of cement and 386 barrels of displacement were proposed.
