0357701494 2019-02-07 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that there were 5 existing wells on the pad. The wells were all equipped with 13.375-inch diameter casing and the A section of the well head. At the time of my inspection all the cellars were holding water. Vegetation had not been re-established around the pad and there were no annular pressures to monitor. There was proper signage at the entrance to the lease road and the well had proper identification.
0714976358 2019-01-09 Casing String Witnessed No Violations On the day of my inspection I witnessed that Falcon Rig 26 had drilled the 17.5-inch diameter hole to a depth of 1555 feet. Wyoming Casing crew ran 1548 feet of J55 Buttress thread casing at 61 pounds per foot with 2.6 feet of casing above Kelly Bushing, which is 13 feet above ground level. C&J Energy Services was on site and rigged up their cement head and loaded lines with 5 barrels of fresh water and pressure tested to 2500 pounds per square inch. C&J Energy Services pumped a 35-barrel gel spacer, washed up with 5 barrels of fresh water and then pumped 324 barrels 1529 sacks of Class A cement at 15.6 pounds per gallon with a yield of 1.19 cubic feet per sack, mixed with 5.23 gallons of water per sack. Cement was displaced with 228 barrels of fresh water, plug was bumped at 680 pounds per square inch and pressured up to 1235 pounds per square inch floats held with .5 barrels of flow back. There was 58 barrels of good cement to surface. I informed Ascent Resources Company man Tony Shepard that the casing must remain static until cement reaches a compressive strength of 500 pounds per square inch.
1211142654 2019-04-24 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Akita Rig 802 was currently drilling the 8.5-inch diameter lateral section of the well bore at 23,595. After speaking with Ascent Resources consultant John Wilson, he informed me that they intend to drill the 8.5-inch section to approximately 25,725 feet, he believes they will reach the total depth of the well bore on 4/25/2019 and will be cementing the 5.5-inch diameter production casing on 4/27/2019. I informed John Wilson to keep me updated with the progress of the well and performed a walk around inspection of the rig and found no issues that needed addressed.

1496851592 2019-04-17 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Akita Rig 802 was currently drilling in the 12.25-inch diameter deep intermediate section of the well bore at 6,975 feet. After speaking with Ascent Company man John Wilson, he informed me that they plan to drill to an estimated total depth of 7,400 feet. John also informed me that they would most likely be cementing on 4/18/2019. John will keep me informed on the progress and I will follow up at later time. I performed a walk around inspection on the rig and did not see any issues that needed addressed.
