0822891668 | 2018-07-18 | Completion Testing | No Violations | On the day of my inspection accompanied by ODNR inspector Bill Stolarik I witnessed that the well was equipped with a four-string casing design and was currently being pressure tested. Keane Pressure pumping, and wireline service was currently on location and had not begun completion operations. The well was rigged up for completion operations but would not commence until the West Unit 5H was completed. According to XTO Company Man The producing wells on the pad had VR plugs installed at the well head. All wells on the pad had annular pressure monitoring in place, at the time of my inspection the 9.625 inch diameter casing gauge was 0 pounds per square inch, the 13.375 inch diameter gauge was at 240 pounds per square inch and 18.625 inch diameter casing gauge read 0 pounds per square inch. Keane was preparing to run their perforation tool into the first stage of the well. |

When tripping the drill pipe out of the hole there was an influx of natural gas coming from the well bore. The gas that was encountered was flared by the company. Appalachian was tripping the drill pipe back into the hole to load it with fluid to create a static environment to perform the cementing procedures. Once a static environment is established the 13.375-inch diameter casing will be ran and cemented into the well bore. |

The 18.625-inch diameter casing was ran to a depth of 628 feet measured from ground level. Bow spring centralizers were utilized to provide proper stand-off from the well bore. C&J pumped 40 barrels of water and tagged it to ensure that the cement would be placed properly in the well bore. The water was tagged just below the floor of the mine. C&J Cementing Services provided the cementing procedures. A safety meeting was held prior to cementing operations to ensure safety precautions were met and go over the cementing procedures. C&J ran 38 barrels of Class A cement with 3 percent Calcium Chloride to tack the shoe. Two 1-inch diameter grout strings were ran in the annulus space. C&J pumped 155 barrels of Class A Cement and cement returns to surface were established. |

XTO Production Superintendent informed me that there was 7600 pounds per square inch on the 5.5-inch diameter casing. Annular pressures were being monitored with gauges and at the time of my inspection the 9.625-inch diameter casing gauge read 0 pounds per square inch and the 13.375-inch diameter casing press was at 700 pounds per square inch. Production superintendent Both the 9.625-inch diameter casing and 13.375-inch diameter casing were tied to a pressure bleed down line and the 13.375-inch diameter casing was open at the time of my inspection. Production Equipment included a gas processing unit and 6- 400-barrel steel production fluid tanks. There was a Key Energy Services coil tubing rig and pressure pumping truck on location at the time of my inspection. After speaking with XTO Production superintendent Tracy Vanscoy he informed me that Key Coil Tubing Rigs blow out preventer malfunctioned, and they were currently waiting on a new Blow Out Preventer which is scheduled to arrive on Friday 10/19/2018. |
