0123368418 2018-08-22 Completion Testing No Violations Status check inspection found the well drilling process is now complete. The well head configuration consists of a Cactus Company lower A section that is attached to the 13.375 diameter casing string, this is a new style well head system that the 9.625 and 5.5 diameter casing string land out in the A section by the means of mandrill hanger systems. The B section well head assembly consists of the 5.5 production casing flow cross only. The company has removed the lower and upper completion master valves and installed a service cap and 2 butterfly style valves to allow monitoring and pressure relief of the internal of the 5.5 diameter production casing which has not had the toe sleeves open or the initial set of perforations shot in the casing. A company identified as Full Circle is on site installing riser pipes on the well head A sections to monitor annular pressures. 7 wells are present on location and no production equipment is present on location. A mid stream pipeline is being installed to the pad midstream service point. 7 wells are present and have been drilled on the pad. The rig that drilled the wells identified as H&P 628 is disassembled and stacked out on location.
0234884087 2017-09-17 Drill / Deepen / Reopen No Violations Status check inspection on the well drilling process found H&P Drilling rig 628 on location and currently drilling the 24 diameter surface borehole at measured depth of 322. The company is drilling with a tri-cone bit and using fresh water circulating at 644 gallons per minute with the drilling rigs mud pumps with a rate of penetration of 8 to 15 per hour. There is a mine void from 420 to 430 previously identified while drilling the # 7 well which was plugged and abandoned due to well bore issues. The company will drill past the mine void to a proposed depth of 490 and set 18.625 casing to a point of approximately 50 below the mine void floor. They proposed casing tally shows a hydraulically inflated swell packer to be set in the 18.625 string at 388.85. The well bore is currently holding a hydrostatic Colum of fluid to surface and not loosing any fluid to the formations. The Company has indicated they will apply for a permit change to the well identification from the # 6 well to the # 7-A well and so forth respectively to the remaining 5 wells spudded with 26 diameter conductor set with the end result of adding a new #1 well on the opposite end to replace the # 7 well that was plugged.
0486761050 2017-10-02 Casing String Witnessed No Violations H&P 628 had reached a total depth of 466 feet in the 24-inch diameter open hole section of the well bore. During drilling operations, a void was encountered from 407 feet to 418 feet within the open bore hole. The 18.625-inch diameter casing was ran into the open hole to total depth. The casing equipment included an inflatable packer that was installed from 356 feet to 383 feet on the casing string. Stingray Cementing Services was on location to cement the casing in place. Inspector Bill Stolarik was on site also to witness the installation of the casing string. See his report for further detail regarding the cementing process.
0746089608 2019-03-20 Completion Testing No Violations Status check inspection found the well drilling and stimulation process complete on all 7 wells on the pad. Annular pressure monitoring is installed on each well on the pad with no pressures noted on any well above 500 psi. The tin horn cellars are free from standing fluid. A 3rd party contractor identified as shale pro was on site installing the surface production equipment, all sub surface equipment was previously installed. There are some areas of disturbance around the well pad perimeter where the mid stream operator installed the mid stream pipe line and the fresh water flow line was located during stimulation, these areas will need vegetation established. The company has a service rig on the neighboring Ridgetop pad and once drill out is complete there the rig will move over to the said well pad.
0841430759 2017-09-20 Casing String Witnessed No Violations On site to witness a hydrostatic test on the 18.625-inch diameter mine/surface casing string. The operator drilled 10 feet below the 18.625-inch casing string and filled the hole with fresh water. The hydrostatic test was held for a 30-minute time period and the test was successful. The water level did not drop within the casing and the operator was permitted to resume drilling operations.

Mouse hole was also concentrically drilled. Cellars are to be cemented tomorrow morning. Pad address is 57084 Nixon Run Road, Martins Ferry Ohio 43935. Spudded 8/27/17.
1003005473 2019-05-02 Completion Testing No Violations Status check inspection found 7 wells present on the well pad. All 7 wells have been drilled and completed and the completion plugs drilled out. The completion rig has finished up and is off location. All 7 wells are equipped with 2 primary master valves and will produce off the 5.5 diameter production casing. Flowback has not moved onto location and began rigging up yet.

1505774734 2017-09-26 Casing String Witnessed No Violations On site inspection to witness the 18.625 casing installation and cement job with H&P Drilling -Stingray Cement Service and Wyoming Casing. The rig has drilled a 24 diameter surface borehole on fresh water to a measured depth of 471 off of a 28.6 drilling rig Kelly Bushing heigth with the mine void being encountered between the depths of 407 to 415 with total fluid loss at 405. Upon reaching total depth the rig pulled the drill string and began their casing run inserting 473 of 18.625 diameter J-55 grade buttress thread casing with a guide shoe and float collar ran along with a Wetherford Swell Packer isolation tool set between 389 and 362. The company were initially unable to get the casing in the hole past 424 on the 1st day and had to pull their casing to clean out the borehole below the mine void.
1505826344 2017-09-19 Casing String Witnessed No Violations Status check inspection and day 2 on location to witness the 18.625 diameter surface mine string 2 stage cement job. Upon arrival I witnessed H&P Drilling rig 628 on location and found that overnight the rig had pulled the 18.625 casing set in the well on the previous day out of the hole and ran back in with a 24 insert bit and blew the hole clean on air. Casing was then re installed in the well bore with casing being set to a depth of 473 and the swell packer set with the bottom of the packer element at 389 and top of the element at 362, the casing string float is at 430. The rig loaded the casing with fluid capacity plus 10 barrels before pumping the bottom balance job to seal the section of casing below the mine void. The cement pumped was a 30 barrel 15.6 pound per gallon slurry consisting of 140 sacks of class A cement. The cement was pumped and displaced with 131 barrels of fresh water. Upon the landing of the cement plug at 3 barrels a minute and 8 psi the casing pressure was increased to 550 psi in attempt to pressure test the casing however due to the slow displacement rate of 3 barrels a minute the cement truck pumps had lost their prime and the final pressure test and packer inflation was turned over to the drilling rig. At that point the drilling rig hooked up to the cement head and pressure tested the casing string to 530 psi with no recorded loss over 10% in 5 minutes. The pressure was then increased to 730 psi and then 975 and finally increased until the packer deployed at 1,172 psi and pressure dropped back to 975 confirming the packer opened. At that point casing pressure was increased to 1,200 psi for a final casing and swell packer pressure test. At that point the casing slips and rotating head seal was pulled and a measuring device was lowered into the annular space to confirm the well bore was open to the swell packer and had no fluid above it with a final tag at 352 and swell packer top at 362. The well was then topped off to surface with
1555006213 2019-04-11 Completion Testing No Violations Status check inspection found 7 wells present on the well pad, all 7 wells have been drilled and stimulated. A service rig owned by Hybrid Energy is rigged up on the said well and has drilled out all the completion plugs down to total depth inside the 5.5 diameter production casing. The rig has pulled the 2.875 workover string back up hole inside the 5.5 to the top of the horizontal curve section to a measured depth of 11,000. There is a tubing snubbing unit owned by Axis rigged up on the (2) primary master valves and the tubing will be snubbed out of the hole from the current depth 11,000 to surface. No production tubing will be set in the well bore. The well will be produced initially off the 5.5 production casing.

The last formation top marked was the Queenstown at 8,340 True Vertical Depth.
1721409753 2019-01-02 Completion Testing No Violations On this day I attend the pre stimulation safety meeting along with all current contractors and company representatives. 7 wells are present on the pad, all well have the drilling process complete and are rigged up to be stimulated. The stimulation company is Stingray Pressure Pumping Service and the wireline is Renegade Wireline along with Select Energy Flowback. The well head A & B Sections are Cactus with the completion stack being from B&B. The well head B section which is the 5.5 x 9.625 annular space does have pressure control attached with electronic transducers in place to monitor pressure. Transducers are also attached to the A section which monitors between the 9.625 and 13.375 well head section. The well pad surface is of stone construction with earthen berms in place.
1939269775 2018-02-20 Completion Testing No Violations On the day of my inspection the site was still being occupied by H&P Rig 628. Although the rig was on location it was laid down and await mobilization off site. The trailers where the employees stayed were still on location. The well was equipped with 5.5-inch diameter production casing and was shut in. The well is awaiting completion operations at this time.
2020943315 2017-10-03 Casing String Witnessed No Violations On this day, the H&P rig 628 conducted a hydrostatic casing shoe test. The rig drilled from 466 feet - 471 feet which was 5 feet below the 18.625-inch diameter casing string. The casing was loaded and a 30-minute waiting period ensued. The test was successful because the fluid level remained at surfaced during the 30 minute period.
