0899623854 2018-11-14 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well site construction has been restored with vegetation established and no erosion found. The erosion control filter socks have been removed. The well pad surface and lease road is stone construction and in good repair. The well site premier boundary drains are in the closed position. A mid stream operation is present along the lease road at the left front corner entry point of the well pad. Emergency shut down devices and fire extinguishers are present at all critical areas of the well pad. Signage and hazard warning labels are posted. There are 5 producing wells in production on the pad with annular pressure monitoring in place on each well head A&B section consisting of 2 riser pipes equipped with valves and gauges, both electronic and manual. The 13.375 diameter casing well head (A) section gauge read 325 pounds pressure and the 9.625 diameter casing well head (B) section gauge read 750 pounds pressure. The 5.5 diameter casing internal pressure gauge read 3,000 pounds pressure and the flow line down stream of the production tree choke valve assembly read 1,250 pounds pressure. The well head is equipped with emergency shut down devices.

There are mats and jersey barriers currently stacked out on the side of the location. There is a pipeline beside the pad that is waiting on completion operations to commence and production from this well to begin.

1493840022 2017-05-03 Casing String Witnessed No Violations Status check inspection to witness the conductor casing cement job with Rocky Mountain Drilling and Stingray Cement Service. Rocky Mountain Drilling has drilled a 32 diameter borehole to a measured depth of 110 and ran 26 diameter .375 wall casing to 110. Stingray cemented the casing using a 56 barrel cement mix consisting of 260 sacks of class A cement with the cement weight verified by weight of scale. The cement was displaced to 100 with 62 barrels of fresh water. Cement returns were witnessed to surface.
1499821835 2017-07-11 Casing String Witnessed No Violations On site inspection to witness the mine string cement job with H&P Drilling rig 628 on location along with Stingray Cement Service. The rig has drilled a 24 diameter borehole to a measured depth of 623 off of a drilling rig Kelly bushing heigth of 26.80. The mine void rubble zone was encountered at 570 to 578 determined by loss of weight on the drill bit. Circulation to surface was lost at that point also. The rig proceeded to pull the drill string and rig up Wyoming Casing Services who ran the 618 of 18.625 diameter J-55 grade buttress thread casing with a casing guide shoe ran on bottom and a float collar set in the string at 576. A Wetherford manual deployed hydraulically inflated swell packer was set in the string at a measured depth of 534 with the packer pressure shear pins set to inflate at 1,300 pounds per square inch (psi). Stingray Cement Service cemented the casing in a 2 stage process with the casing shoe track cemented below the mine void by using a 35.3 barrel cement mix consisting of 167 sacks of class A cement weighing 15.6 pound per gallon with the cement weight verified by weight of scale, the cement yield used was 1.19. Upon the pumping of the cement the cement plug was dropped and the cement displaced with 179 barrels of fresh water. The cement final displacement pressure upon the landing of the plug was 200 psi. Upon the landing of the plug the casing was pressure tested at 662 psi and the pressure increased until the swell packer inflated at 1,300 psi. Proceeding the deployment of the swell packer the annular void was topped off from surface with 136 barrels of cement.
