1507131536 2017-10-04 Casing String Witnessed No Violations physical address: 62827 johnson south road, bethesda oh 43719. On the day of my inspection I witnessed Express energy had augured a 36 inch diameter hole to 100 feet. 100 feet of 30 inch diameter conductor casing was run. Express was onsite with 275 sacks of class A cement. Additives include 2% CaCl, and .25 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. 50% percent excess ran. Cement was displaced with 70 barrels of fresh water. 15 barrels of cement returned to surface.
1522330279 2018-03-26 Drill / Deepen / Reopen No Violations H&P Rig 471 was drilling in the 17.5-inch diameter open hole section of the well bore at 1550 feet. Once the total depth of the well is reached the company will run the 13.375-inch diameter casing string into the well bore. B&J will perform the cementing procedures for this casing string.
1567352799 2017-09-28 Drill / Deepen / Reopen No Violations Express Energy services was on site drilling the cellar for this well bore. During my visit the total depth of 8 feet was reached and the plastic cellar was put into the bore hole and Express Energy grouted the backside of the cellar with cement.
1735738273 2018-10-29 Completion Testing No Violations On site inspection found the well drilling and stimulation process is now complete and crews are on site preparing for the completion drill out process. The Service rig owned by CWS will arrive tonight and rig up on the H-6 well. A company identified as Tecton will handle the flow back operations with the company men indication no wells will be put into flow back until the drill out operations are complete. 56 total stages were complete. The fluid to be used to drill out was weighed and is 9:1 pound per gallon brine from other production operations in the area. The polymer containment that that the service rig will be situated on appears to be in good repair and free of well bore material. The well pad boundary does have a sound barrier in place on 3 sides. There is a above ground storage tank on location that does have bring water in it. The production fluid facility has been erected and consists of a total of 6-500 barrel capacity steel tanks situated inside a polymer lined steel containment dike area. All 3 wells present that were recently completed have annular pressure monitoring in place on the 13.375 and 9.625 well head sections along with an additional gauge present to monitor the well head B section seal area between the 5.5 and 9.625 diameter casing strings.
1985667635 2019-03-04 Production Wells No Violations Status check inspection found 6 wells present on the location with all 6 being drilled, however only (3) have been recently stimulated and are in the flow back production stages. The (3) producing wells are the # 2-#4 and #6 wells. The wells are producing through choke manifold systems into a gas processing unit. The said well is producing off the 5.5 diameter production casing through a primary master valve with a in line emergency shut down valve present between the well head and choke valve. The 5.5 casing on the said well is equipped with an electronic transducer only with no manual gauges present on any portion of the well head to confirm pressure. No annular pressure monitoring is in place on the well head A or B sections. I have informed the company men on site to install annular pressure monitoring on all 6 wells present on the pad. Production equipment on location consists of (4) gas processing units along with a production facility that consists of (6) steel 500 barrel capacity tanks situated inside a polymer lined steel containment dike area. Addressing the preliminary restoration- The well pad surface and lease road are stone surface construction and in good repair. There is a large above ground storage tank on location that is reported to contain fresh water only that will need removed along with the sound wall barrier that is erected on the earthen berm before the restoration can be signed off on.
2027803769 2018-12-20 Production Wells No Violations Status check inspection found the well drilling and stimulation complete and the well currently in the production flow back stages. The well is selling through a sand trap separator and choke manifold into a gas processing unit. The production fluid is going into (2) 500 barrel horizontal frack tanks. (6) wells are present on the pad that have been drilled however only (2) are producing. A sound wall barrier is present around the well pad location and the pad surface along with the lease road are stone construction and in good repair.
