0220401476 2018-12-20 Production Wells No Violations Status check inspection found the well is in the initial production stages and selling off the 5.5 diameter production casing through (2) primary master valves through a choke manifold system. The production fluid is being fed into the tank battery that consists of (5) steel tanks all capable of holding 500 barrels of fluid volume each. The flow back equipment is situated on polymer containment with the well pad perimeter berm serving as the primary containment. The well pad and lease road surface are stone construction and in good repair. 6 wells are present on the pad that have been drilled and (2) have been completed and are in production.
0499224628 2018-05-02 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that H&P 471 was currently drilling the 12.5 inch diameter, deep intermediate section of the well bore. They were currently at 7475 feet deep drilling at approximately 150 feet an hour. Company man Pat Smith informed me that they had just made a trip and had a small flare on their blowdowns. Gas detection equipment was not functioning at the time of the flare, no H2S was detected on portable meters. They plan to drill to an estimated Total Depth of 8400 feet and run casing to Total Depth and cement to surface. Pat Smith also informed me that the 13.375 inch diameter casing was set at 2211 feet and they had cement to surface with approximately 60 barrels excess returned to surface. There were no violations at the time of my inspection.
0625737039 2018-09-19 Completion Testing No Violations On the day of my inspection I witnessed that GR Energy Services was on location and setting isolation plugs for the 21st stage of the well out 56 total stages. Pro Frac was on location when I arrived and had fracturing iron rigged up on the 2H and 6H. I spoke with EQT company man Stephen Vandenberg who informed me that they had fracked 20 stages on the well and were planning to complete this well before they began hydraulic fracturing on the other wells on the pad. Stephen informed me that they would be using 600,000 pounds of sand per stage.

1123259640 2019-03-04 Production Wells No Violations Status check inspection found 6 wells present on the location with all 6 being drilled, however only (3) have been recently stimulated and are in the flow back production stages. The (3) producing wells are the # 2-#4 and #6 wells. The wells are producing through choke manifold systems into a gas processing unit. The said well is producing off the 5.5 diameter production casing through a primary master valve with a in line emergency shut down valve present between the well head and choke valve. The 5.5 casing on the said well is equipped with an electronic transducer only with no manual gauges present on any portion of the well head to confirm pressure. No annular pressure monitoring is in place on the well head A or B sections. I have informed the company men on site to install annular pressure monitoring on all 6 wells present on the pad. Production equipment on location consists of (4) gas processing units along with a production facility that consists of (6) steel 500 barrel capacity tanks situated inside a polymer lined steel containment dike area. Addressing the preliminary restoration- The well pad surface and lease road are stone surface construction and in good repair. There is a large above ground storage tank on location that is reported to contain fresh water only that will need removed along with the sound wall barrier that is erected on the earthen berm before the restoration can be signed off on.

1540830752 2018-10-29 Completion Testing No Violations On site inspection found the well drilling and stimulation process is now complete and crews are on site preparing for the completion drill out process. The Service rig owned by CWS will arrive tonight. A company identified as Tecton will handle the flow back operations with the company men indication no wells will be put into flow back until the drill out operations are complete. 56 total stages were complete. The fluid to be used to drill out was weighed and is 9:1 pound per gallon brine from other production operations in the area. The polymer containment that that the service rig will be situated on appears to be in good repair and free of well bore material. The well pad boundary does have a sound barrier in place on 3 sides. There is a above ground storage tank on location that does have bring water in it. The production fluid facility has been erected and consists of a total of 6-500 barrel capacity steel tanks situated inside a polymer lined steel containment dike area. All 3 wells present that were recently completed have annular pressure monitoring in place on the 13.375 and 9.625 well head sections along with an additional gauge present to monitor the well head B section seal area between the 5.5 and 9.625 diameter casing strings.
