| 0016353934 | 2018-06-04 | Drill / Deepen / Reopen | No Violations | On the day of my inspection H&P 471 had drilled approximately 593 feet of cement and reached a total depth at 2165 feet which was 4 feet above the float collar. When total depth was reached they ran a gyro survey and loaded the hole with approximately 243 barrels of fresh water. The 13.375 inch diameter float collar was set at 2169 feet. Hosco Services was on location to perform a casing integrity test, the 13.375 inch diameter casing was pressured up to 502 pounds per square inch at 12:38 on 6/4/2018. After 5 min pressure had fallen to 496 pounds per square inch, and after the 30 minute test pressure was at 467 pounds per square inch. Company man Pat Smith Informed me that they would continue to drill out the shoe track, and begin drilling the 12.25 inch diameter deep intermediate portion of the well bore. He believed that they would be setting and cementing the 9.625 inch diameter casing on 6/8/2018. |

The number 2 #4 and # 6 wells are in the flow back production stages and are not equipped with annular pressure monitoring either. There is a large above ground storage tank on location. The well pad perimeter does have a sound wall erected on top of the earthen berm. A third party midstream pipe line was installed ant the earthen berm was removed and not properly put back. I did notify ODNR Engineer Technician Anthony Carson on this day of the deficiency related to the berm. |

At the time of my inspection the stimulation process was currently shut down due to a wireline failure at surface. Annular pressures were being monitored on every well on the pad. After speaking with EQT consultant Brad Wolff he informed me that they had a Schlumberger wireline failure at surface, they intend to switch out the Schlumberger wireline truck and tool. I informed EQT consultant Brad Wolff to keep me updated with any problems or situations that occur during the stimulation process. |

Express was onsite with 275 sacks of class A cement. Additives include 2% CaCl, and .5 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.18. 50% percent excess ran. Cement was displaced with 70 barrels of fresh water. 12 barrels of cement returned to surface. |
| 1922781693 | 2018-05-30 | Casing String Witnessed | No Violations | On this day, I am onsite to witness the 18.625-inch diameter surface casing cement job with BJ Services. The H&P FlexRig 471 rig drilled a 24--inch diameter J-55 87.5 lb./ft. casing into the wellbore. BJ Services circulated the hole with 192 barrels of water and pumped 35 barrels of gel spacer, ahead of cement. They cemented the casing with 145 barrels total volume (680 sacks) of Class A cement with a weight of 15.6 pounds per gallon, verified by scale, and a 1.2 yield. It was displaced with 100 barrels of fresh water. The plug landed with 146 pounds per square inch and then pressured up to 670 pounds per square inch. It was noticed that the cement-head had a leak and the pressure declined 25 psi per minute for 5 minutes. The floats held and 1 barrel was bled back. 15 barrels of cement returns were brought to surface. Update on 5/31/2018 Company Man, Pat Smith, informed me that they waited on the cement for 6 hours, and then retested the casing to 300 psi, for 30 minutes, and the pressure held. |
