0561370362 2018-06-21 Completion Testing No Violations Status check inspection found a service provider identified as Keane Energy on location with their pressure pumping units (17 total) along with 2 pump down trucks and a wireline truck. The said well has a total of 44 stages stimulated. The well head completion stack is owned by Cactus and consists of a lower master valve followed by a hydrologic master valve then a flow cross with an additional manual master valve atop the flow cross section. Both the 13.375 and 9.625 well head sections have pressure monitoring devices installed.

0934843960 2018-07-11 Completion Testing No Violations On site inspection accompanied by ODNR Inspector Ryan Armstrong found the well drilling and stimulation complete. The completion drill out process is complete and the well is awaiting the initial stages of flow back operations. In conversation with the company men on site it was discovered that the 9.625 diameter casing well head section is open and hooked into a choke manifold system with no pressure on the 9.625 section. It was also observed that the 13.375 diameter well head section was not being monitored.
1498672351 2017-06-28 Casing String Witnessed No Violations On site inspection to witness the Conductor casing cement job at 13:15 hours along with Express Energy Conductor Drilling rig and Express Energy Cement Service. Express Energys conductor rig has drilled a 36 diameter borehole to a measured depth of 100 from ground level with fresh water encountered at 80 from a coal seam. The rig then inserted 100 of 32 diameter .375 wall casing into the borehole. The casing was cemented with 57 barrels of cement consisting of 275 sacks of class A cement with 2% calcium and .50 pound per sack loss circulation material. Cement calculations were for annular capacity with 50% excess and no shoe track, the cement weight was verified at 15.6 pound per gallon by weight of scale. The cement was displaced to 90 with 75 barrels of fresh water. Cement returns were witnessed to surface with 1 of cement left inside the cellar floor to create a liquid tight seal.
1509546083 2017-11-01 Drill / Deepen / Reopen No Violations On the date of my inspection I found Patterson rig 296 currently running their 5.5 inch production casing at approximately 16700 feet. Rice Energy expects to be cementing this string this evening. They will then be moving the rig to a different pad. The containment around the two chemical storage areas have multiple tears and punctures. The Rice Energy company man indicated that they have a contractor scheduled to come out on 11/2/17 to work on their containment. All of the chemicals were within containment. The pad is surrounded by an earthen dike covered by healthy vegetation. The pad drain valves are in the closed position and do not appear to be leaking.
1910642569 2017-08-01 Casing String Witnessed No Violations On location to witness the cementing of the surface casing. The rig ran 20 casing and all the required float equipment to 503. O-Tex Pumping cemented it with 770 sacks of Class H cement. Good cement was returned to the surface, the pressure test of the casing passed. See the casing ticket for the details.
1915247987 2019-03-28 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well site meets ORC 1509.72 with the area affected by the well site construction restored, vegetation is established and erosion control is in place. The well pad surface and approach is stone construction and in good repair. 5 wells are present on the pad and in production, all wells are selling off the 5.5 production casing through (2) primary master valves with emergency shut downs in the production outlet flow line. The wells are selling through sand trap separators with a temporary choke system attached to the sand trap separator. The sand trap separators are being evacuated into a single 500 barrel steel horizontal tank set in polymer containment. Permanent production equipment includes (5) gas processing units along with a production facility consisting of (6) steel 400 barrel tanks set inside a polymer lined steel containment dike area. Also present is a 3rd party mid stream facility.
