0160395166 2018-08-29 Production Wells No Violations Status check inspection found 3 wells present on the pad all in production. The lease road has proper identification posted at the entry point and has a gate across it at the well pad entry point. The lease road and well pad are of stone surface construction and in good repair. The well pad perimeter is protected by an earthen berm. There is a Mark West mid stream pipeline station along the lease road at the right front corner of the well pad. There is 3 gas processing units on the left side of the pad and the tank battery is at the left rear corner. The tank battery consists of (4) steel construction 400 barrel capacity tanks and one 50 barrel steel tank, all situated inside a polymer lined steel containment dike area. Hazard warning labels are posted on the tank and there is emergency shut down devices situated at all critical areas on the well pad including at the entry point gate. The well head assembly consists of a lower A section and upper B section with 2 diameter riser pipes attached to both section outlet ports with valves and gauges present to monitor annular pressure. The A section gauge which monitors the pressure between the 9.625 and 13.375 diameter casing string reads 290 pounds pressure. The B section gauge which reads the pressure between the 5.5 and 9.625 diameter casing strings reads 480 pounds pressure. The 5.5 diameter casing internal pressure gauge reads 800 pounds pressure. There is tubing in the well and the tubing head gauge reads 750 pounds pressure and the production flow line downstream of the choke reads 200 pounds pressure, the well is open flowing up the tubing. Identification is present on each well head.

The well head completion consist is made up of a Cactus Well Head B section and Tubing head assembly rated at 15,000 pound per square inch working pressure (psi) with a Red Dirt master valve isolation tool on top of the well head followed by a manual master valve and then 3 hydraulic master valves. The well bore production casing is 5.5 diameter and consists of 4 pressure activated sleeve assemblies ran in the 5.5 production string between the depths of 17,419 and 17,320. The sleeves have been opened and the 1st stage stimulated. The well bore true vertical depth is 8,609. All stimulation equipment is situated on polymer containment with the well pad perimeter berm serving as the primary containment.

0989669536 2018-06-19 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5 inch diameter production casing that was open flowing. Annular pressure monitoring was in place, the intermediate casing gauge read 400 pounds per square inch, the surface casing gauge read 290 pounds per square inch. Production equipment included a Sand Separator, Gas Processing unit, 4- 400 barrel purge tanks, and a 50 barrel produced water tank shared with 34013211840000, 34013211860000. Identification tags were present, and production was up to date. No violations were found at the time of my inspection.

The rig is drilling on air at 5,850 cfm with a proposed total depth to be drilled is 1,514.
1491942566 2017-04-11 Casing String Witnessed No Violations On site inspection to witness the surface casing cement job accompanied by Inspector Andrew Corder. Upon inspection H&P rig 628 was on the hole along with Stingray Cement Service and Wyoming Casing Service. The rig has drilled a 17.5 diameter surface borehole on air with a rotary hammer bit to a measured depth of 1,514, upon reaching total depth the rig pumped a 40 barrel gel sweep and circulated the total hole with fresh water behind the gel sweep with circulation established to surface. The rig then pulled the drill string and ran a borehole deviation survey. Upon completion of the survey the rig proceeded to run 1,512 of 13.375 diameter J-55 Grade Buttress thread casing with a casing guide float shoe and float collar casing equipment ran along with 12 casing centralizers being placed on the casing every 4th collar. Stingray Cement Service cemented the casing with a 282 barrel class A cement mix consisting of 1,366 sacks of cement. The cement weight was verified by weight of scale. The cement was displaced with 228 barrels of fresh water and the casing was pressure tested to 1,245 psi which was 500 psi over cement displacement pressure of 715 psi. With 60 barrels of cement returns to surface.
