0694861586 2018-08-08 Production Wells No Violations On site inspection accompanied by ODNR Inspector Ryan Armstrong found the well drilling and completion process now complete. The well is in the initial flow back production stages flowing trough a sand trap separator and choke manifold system, there is a 500 barrel horizontal tank present the sand trap separator is being discharged into. Annular pressure monitoring is not in place on the well head A or B sections.
1514323623 2017-12-26 Casing String Witnessed No Violations On site inspection to witness the surface casing cement job with Patterson Drilling rig 582 and Baker Hughes Cement Service. The rig has drilled a 24 diameter surface borehole to a measured depth of 485 off of a drilling rig Kelly Bushing heigth of 25. The rig inserted 453 of 20 diameter 94 pound per foot J-55 Grade Buttress Thread casing. Baker Hughes cement service cemented the casing by first pumping 165 barrels of fresh water followed by 35 barrels of gel spacer than another 10 barrels of fresh water before starting the 168 barrel cement mix consisting of 871 sacks of class A cement displaced with 147 barrels of fresh water and 20 barrels of cement returns to surface.
1535389412 2018-08-27 Production Wells No Violations On the day of my inspection I witnessed that the well was currently in flowback operations with 2 select flowback sand traps, and a choke manifold, there was also two 500-barrel fluid storage tank hooked up to flowback equipment. The well appeared to have gauges monitoring annular pressure but were underneath water and I was unable to read them. Production equipment included an individual Gas Processing unit and 4- 400-barrel production fluid tanks, and a 100-barrel production fluid tank shared with the other producing wells on the pad. The well pad and all equipment were within earthen containment with secondary containment around the production tanks. There were multiple Emergency Shutoff Devices on location, Identification tag were present, and production was current.
1541439607 2018-11-05 Preliminary Restoration No Violations Preliminary Restoration status check inspection finds the well site meets ORC 1509.072 with vegetation established and erosion control in place. The well pad and lease road surface is stone construction and in good repair. There are 5 wells total present on the pad with the # 2 & #4 being previously drilled and the #1-#3 &#5 being drilled in the 2nd round. The said well has no annular pressure monitoring or identification posted and is being flowed through a vertical sand trap separator with the fluid being ran into a temporary storage area consisting of (2) 500 barrel horizontal tanks. There is 3 gas processing units present on the pad. The permanent production fluid storage area consists of (4) steel tanks capable of holding 400 barrels fluid volume each and one horizontal 100 barrel tank, all situated inside a polymer lined steel containment dike area.
1777357780 2018-06-21 Completion Testing No Violations Status check inspection on the well drilling process found the well completed and awaiting to be put into the initial flow back production stages and will produce from the production casing through the completion stack. The stack is owned by Silver Creek and consists of 2 primary manual master valves and a hydraulic master valve. No production tubing will initially be set in the well bore. The well is selling into a gas processing unit. No annular pressure monitoring is in place on the well. Also on site is a production facility consisting of 4 tanks of steel construction design situated inside a polymer lined steel containment dike area. There are 2 wells on the pad that were previously drilled and completed identified as the Iron Warrior # 2 and # 4 wells. 3 new wells were drilled and are in the initial flow back production stages being produced through choke manifolds and sand trap separators, these wells are identified as the Iron Warrior # 1-3 & 5 wells.

Production equipment included an individual Gas Processing unit and 4- 400-barrel production fluid tanks, and a 100-barrel production fluid tank shared with 34013209590000, 34013209580000, 34013211720000, 34013213130000. The well pad and all equipment were within earthen containment with secondary containment around the production tanks. There were multiple Emergency Shutoff Devices on location, Identification tag were not present, I tried to contact EQT production superintendent Jared Lowery to inform him that they needed to be monitoring annular pressures and wells needed to be identified.
