0262760228 2018-01-09 Production Wells No Violations This well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit and then to a production area. This report shall serve as a correction of the previous inspection report that stated there are two 400-barrel holding tanks. Upon further review and evaluation, the production area contains two 500-barrel production fluid storage tanks. The production reports for this well are current to 2017.

1728538025 2016-12-16 Casing String Witnessed No Violations I am on site today to witness the cementing of the 30 inch diameter conductor casing of the well bore. Precision Rat hole drilled a 36 inch diameter hole to a depth of 100 feet then proceeded to run in 100 feet of 30 inch diameter casing. Allied Cementing Services is the cement contractor on site. Prior to pumping cement, Allied sent 5 barrels of fresh water mixed with flake. After the fresh water was pumped Allied then pumped 85 barrels of Class A 15.6 pound per gallon, 1.18 yield cement and displaced with 75 barrels of fresh water leaving 10 foot of cement inside the casing. Over the duration of my inspection I witness approximately 15 barrels of good cement returns to surface.
