0212233287 2019-01-09 Production Wells No Violations Status check inspection found 4 wells present on the pad and in production. All 4 wells are producing off the 5.5 diameter production casing through 2 primary master valves with an additional emergency shut down valve in line. The wells are selling through 3rd party choke manifolds and sand trap separators. A separate facility is present with 4 steel 500 barrel tanks set inside polymer containment, these tanks are being used for the sand trap separators to be evacuated into. No riser pipes or gauges are present on the well head A&B sections to allow for annular pressure monitoring. Additional production equipment includes 4 gas processing units along with a production facility consisting of (4) steel tanks capable of holding 500 barrels fluid volume each. There is an additional horizontal 100 barrel tank inside the tank battery. All tanks are situated inside a polymer lined steel containment dike area.
0781090569 2017-10-25 Casing String Witnessed No Violations I am on site today to witness the cementing of the 18.625 inch mine string casing. H&P rig 571 drilled a 22 inch diameter hole to a depth of 633 feet and set 609 feet of 18.625 inch, 87.5 pound per foot casing. O-Tex pumped 220 sacks of class H cement, 1.21 yield, 15.6 pound per gallon, 3 percent Calcium Chloride, 5.22 gallon per sack water requirement. This was pumped as a balance job so they did not displace the cement. A coal seam was identified from 510 feet to 523 feet and a void was identified from 537 feet to 551 feet, two baskets were set at approximately 507 feet. O-Tex pumped 8 barrels (37 sacks) of class H, 1.2 yield, 15.6 pound per gallon cement to set the basket. After waiting approximately 3 hours O-Tex pumped an additional 58 barrels (269 sacks). After a total of 3 top outs (385 sacks), cement returned to surface and remained static.
1290536088 2017-11-01 Drill / Deepen / Reopen No Violations On the date of my inspection I found H&P rig 471 currently drilling their intermediate string at approximately 7834 feet. Rice Energy expects to reach their total depth for this string on 11/2/17. I found multiple areas around the rig containment that had tears and punctures. The Rice Energy company man indicated that he would have a contractor come out on 11/2/17 to work on their containment. Also, there is a corner of the containment near the pipe rack where it is draining onto the pad. They have been hauling water from this location but cannot keep up with all of the recent rain. All of the chemicals except for one pallet of bulk barite is within containment. The company man said he would move the pallet. The pad is surrounded by an earthen dike covered by healthy vegetation. The pad drain valves are in the closed position and do not appear to be leaking.
1508766905 2017-10-23 Casing String Witnessed No Violations I am on site to witness the cementing of 24 inch surface casing. H&P Rig 471 drilled a 27 inch diameter hole to a depth of 325 feet and set 300 feet of 24 inch 79 pound per foot casing. O-Tex established circulation with fresh water, pumped 35 barrels of bentonite gel spacer, then pumped 465 sacks of class H cement (1.21 yield and 15.6 pound per gallon, 3 percent calcium chloride, 1 pound cello flake per sack). The wiper plug was released and O-Tex displaced with 135 barrels of fresh water. By the time the plug landed approximately 15 barrels of cement had returned to surface. When the plug landed, pressure was increased to approximately 300 pounds per square inch (psi) to ensure casing integrity, the casing held approximately 300 psi for 5 minutes.
1509064190 2017-10-26 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed H&P 471 had drilled a 17.5 inch diameter open hole to a depth of 643 feet. 18.625 inch diameter mine string was set at 633 feet. They drilled 10 feet of new hole and loaded it with water. Wellbore held fluid from 9 to 9:30 am. Hydrostatic test was successful and they are permitted to continue drilling.
1529527601 2018-06-20 Completion Testing No Violations Follow up inspection to a incident reported on 06/19/2018 that involved a Frack Tech pressure pumping truck. The initial incident was reported to occur at 12:47 pm. My investigation that the company was stimulating stage 40 of 63 total stages to be completed when one of the pump engine turbo charger oil seals failed allowing engine oil to enter the engines exhaust system resulting in the oil to catch fire and burn until the engine was shut off and the fire extinguished. The fire was contained to the unit. A minor amount of oil was reported to of escaped the engine and fell upon the containment but was immediately recovered. The pump unit was immediately removed and replaced and was not present on location at the time of my inspection. The company is currently simultaneously completing 2 of the 4 wells present on the pad identified as the #5-H and 7-H wells. The stage of operation during the event (stage 40) was perforated by FTS Frack Tech Services between the depths of 15,411 and 15,249 with the completion plug being set at 15,420. The well head completion stack is rented from Cactus and is 7&1/16 design and consists of a lower manual master valve then a hydraulic master valve followed by a flow cross then a second manual master valve. The 5 x 9 annular is open and has iron rigged into the flow back system with electronic transducers present. The 9 x 13 was not being monitored, I informed the company men on site all annuli shall be monitored according to 1501:9-1 of the well construction rules as described in section (N) addressing annular pressure. All stimulation and support equipment is placed on polymer containment with the well pad perimeter earthen berm serving as the primary containment.
1916051664 2017-08-17 Casing String Witnessed No Violations On the day of my inspection I witnessed Express energy had augured a 36 inch diameter hole to 100 feet. 100 feet of 30 inch diameter conductor casing was run. Express energy was onsite with 275 sacks of class A cement. Additives include 2% CaCl and .5 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. 50 percent excess ran. Cement was displaced with 74.8 barrels of fresh water. 15 barrels of cement returned to surface. No water encountered during drilling.
