0485701166 2018-04-09 Production Wells No Violations The well is equipped with 5.5-inch diameter production casing. The well is currently in the flow-back stage of production. The well is connected to a sand trap. There are ten 500-barrel tanks that are lined up next to the well that are being utilized for the flow-back operation. This well has a gas processing unit on site as well. The production area on this site contains four 400-barrel production fluid holding tanks. There is also a 500-barrel tank next to the production area. Production records within RBDMS are current to 2017.

The 30 casing was ran and cemented to surface by Baker Hughes. Initial cement job did not produce returns to surface. They ran 1 tubing and topped off to surface. Cement to surface established.
1315439249 2016-12-24 Casing String Witnessed No Violations On location to witness the cementing of the 20 casing. The rig ran the 20 casing with all the required float equipment to 442. O-Tex Pumping cemented it with 745 sacks of Class H cement. Good cement was returned to the surface. The pressure test of the casing passed. See the casing ticket for the details.
1667982131 2018-08-15 Production Wells No Violations On the day of my inspection I witnessed that the well was currently in flowback operations with a sand trap, and a choke manifold and ten 500-barrel portable fluid storage tank hooked up to flowback equipment. Annular pressure monitoring was in place on both the 9.625-inch diameter casing gauge read 336 pounds per square inch, the 13.375-inch diameter casing gauge read 1235 pounds per square inch. Production equipment included an individual Gas Processing unit with 2 additional sand traps, and 4- 400-barrel production fluid tanks shared with the other producing wells on the pad. The well pad and all equipment were within earthen containment with secondary containment around the production tanks. There was also an additional 500-barrel portable fluid storage tank on location that was not hooked up to any other equipment and did not have secondary containment around it. There were multiple Emergency Shutoff Devices on location, Identification tag were present, and production was current.
1966323729 2018-09-26 Preliminary Restoration No Violations Preliminary restoration found the well site meets ORC 1509.072 with vegetation established and erosion control in place within the effected areas of the well site boundary. The lease road and well pad are stone surface construction and in good repair. There is 5 wells on the pad that are now in production selling off the production casing with no mechanical or artificial lift technologies present. The sales line pressure at the production tree is 1,142 pounds pressure. There is annular pressure monitoring present with 188 pounds pressure on the 13.375 diameter casing well head section and 188 pounds pressure on the 9.625 diameter well head section. The production fluid facility consists of 4 steel tanks capable of holding 400 barrels fluid volume each, the tanks are situated inside a polymer lined steel containment dike area. Also present on location is 10 temporary 500 barrel steel tanks that are situated inside a polymer liner, the tanks are empty and not currently in service. Well identification is present along with hazard warning and emergency shut down devices at all critical points on the well pad.
