0299668381 2016-08-25 Casing String Witnessed No Violations I am onsite to witness the cementing operations on the second intermediate casing string. Patterson Drilling rig 296 installed 8556 of 40 pound per foot 9.625 casing prior to my arrival. O-Tex Pumping LLC cemented the casing in place with Low Dense 2% Gypsum, 0.21% C-20, 0.5% Fl-4, 0.2% C-51, 0.2% X-Air P, 0.2% X-Static lead cement and Premium H 5% KCl (BWOW), 0.6% C-12, 0.2% X-Air P, 0.1% C-51, 0.1% C-35, 0.1% C-20. I witnessed cement returns to surface.
0302615637 2018-08-21 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a 4-string casing design and was in normal production. Annular pressure monitoring was in place on both the 9.625-inch diameter casing gauge read 1225 pounds per square inch, the 13.375-inch diameter casing gauge read 820 pounds per square inch. Pressure release lines have been installed from the 9.625-inch diameter casing to the production fluid tanks, the 13.375-inch diameter casing had pressure release lines that vented to atmosphere. Production equipment included an individual Gas Processing unit, and 3- 400-barrel production fluid tanks shared with the other producing wells on the pad. The well pad and all equipment were within earthen containment with secondary containment around the production tanks. There were also 4 500-barrel mobile fluid storage tanks on location with secondary containment. There were multiple Emergency Shutoff Devices on location, Identification tag were present, and production was current.

0363032501 2016-08-17 Casing String Witnessed No Violations I am onsite to witness the surface casing cementing operations. Patterson Drilling installed 472 of 94 pound per foot 20 casing. O-Tex cemented the casing in place with 680 sacks of class A cement with 2% gypsum and 2% CaCl. Circulation was established prior to placing the cement. I witnessed 25 barrels of cement circulate to surface. The casing integrity test was performed immediately after landing the latch down wiper plug.
0510997987 2018-04-09 Production Wells No Violations The well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit. From there, it is routed to a production area that contains three 400-barrel production fluid holding tanks. Annular pressures read on the gauges provided appeared to be 1820 pounds on the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing. There was no pressure rating gauge on the inside of the 13.375-inch diameter casing and outside of the 9.625-inch diameter casing string annulus. The production records within RBDMS are currently updated to 2017.

0687757509 2016-09-21 Drill / Deepen / Reopen No Violations My inspection today finds the drilling contractor has drilled an 8.5 borehole to a depth of 20,257. The drilling rig Patterson 296, is breaking down drill pipe and laying it down in preparation for moving off of the site. The site drainage valves are closed due to elevated chlorides. Rice Drilling EH&S is monitoring the site and removing all water. Once the rig is moved, the company will remove chloride contaminated soil from the site.

2114813643 2017-12-19 Production Wells No Violations On the day of my inspection I was accompanied by inspector Andrew Corder. We came to view the pad after the longwall panel had mined passed the pad on the southern side. There did not appear to be any surface damage from mine subsidence to the pad. Gauges read in a clock wise rotation around the wellhead started at the 12 oclock position are as follows: ? Gauge broken, 1000, 0, 3500, 2500. Gauges are not labeled and cellar is full of river gravel. Production equipment includes two GPUs, a line heater, three 400 barrel steel tanks and a vertical seperator.
