0015928524 2016-03-02 Drill / Deepen / Reopen No Violations On this day I checked the pressure test charts from the 18.625 casing job. The charts indicated that the test conducted was succesful. Drilling of the 17.5 open hole section will commence.
0149239676 2018-08-22 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a 4-string casing design with 5.5-inch diameter production casing that appeared to be open flowing. Annular pressure monitoring was in place, the 9.625-inch diameter intermediate casing gauge read 490 pounds per square inch, the 13.375-inch diameter surface casing gauge read 400 pounds per square inch. Production equipment included a Sand Trap, a line heater, and vertical fluid separator. There are two 500-barrel production fluid tanks shared with the other well on the pad. There were multiple Emergency Shut Down devices on location, Identification tags were present, and production was up to date. No violations were found at the time of my inspection.
0191461272 2018-12-26 UrbanProduction Wells No Violations Status check inspection found (2) wells present on the well pad. Both wells are equipped with annular pressure monitoring on the A&B section. The A well head section gauge reads zero pounds pressure and the B section gauge reads 600 pounds pressure. There is production tubing in the well with 1,000 pounds pressure on the 5.5 diameter production casing. The production tubing flow cross has 900 pounds pressure and the production tubing has 800 pounds pressure downstream of the well head choke. Emergency shut down master control valves are present on the well head production tree with shut down control points on all critical areas of the well pad. Both wells are producing with the production valves open on this day. Production equipment also includes (2) line heaters and a gas processing unit along with a contact tower and glycol heater. The production fluid storage area consists of (2) steel 500 barrel fluid volume production tanks situated inside a polymer lined steel containment dike area. The tanks are positive pressure with a relief device installed plumed into a low pressure vapor recovery system. The entire well pad is protected by a polymer liner stretched up over the perimeter berm. The pad is built with a overflow to the East side, the overflow is also lined with polymer and extends downward to a overflow fluid recovery pit area. No standing fluid was in the pit on the day of my inspection. The well pad perimeter is protected by an 8 chain link fence with 3 strands of barbed wire on top. The chain link fence has a double gate at the entry point with signage posted at the entry point along with the well heads and tank battery. There is a second gate at the lease road entrance.

Ascent was directed to the engineering group for approval. I received a call later from Jason Simmerman advising me that is was acceptable for the company to mobilized equipment and set their cellars. The company shall not begin drilling conductors until the pad is certified and they 48 hour grace period is met.
1996593875 2017-12-13 Production Wells No Violations This well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing without any artificial lift technology present at this time. The well is piped to a gas processing unit and then to two 400 barrel production fluid holding tanks. Production records in RBDMS are current to 2017.
