1387564967 2016-09-07 Completion Testing No Violations On site with Austin Gutridge, a follow up for Emergency Response was conducted. The area that had been affected from the brine release appeared to be completely remediated. This well is currently awaiting completion operations.

1470853623 2016-08-10 Drill / Deepen / Reopen No Violations I was on site in response to a call regarding a release that occurred on this pad. Upon arrival I met with some XTO representatives. They explained that the release was due to a mechanical issue. The substance on site consisted of brine water and eco-friendly de-foamer. The area affected measured approximately 40ft*20ft. Following delineation, the company was remediating the area by vacuuming and diluting the affected portion of the pad. The valves on the out flow drains were closed. The company will continue to monitor the sump areas to ensure the clean-up is successful. The remediation efforts will continue until the conductivity and chloride readings reach an acceptable level. This release was within the pad area and did not extend past the berms. Nomac 79 is continuing drilling operations during the remediation.
2115887115 2016-09-12 Completion Testing No Violations An inspection was conducted on this site accompanied by Trish Cartmell. The well is currently awaiting completion efforts. Trish and I reviewed the remediation on site and determined that it was satisfactory.
