1445113578 2015-10-17 Casing String Witnessed No Violations The said well was spudded on 10/17/2015 by Rocky Mountain Drillings to hole rig with a 110 x 32 conductor borehole being drilled and 110 of 26 .375 weld connection conductor being set. Stingray Cement Service cemented the casing using 27o sacks of Class A cement with circulation established prior to cementing and the cement displaced to 100 with 62 barrels of fresh water with good cement returns witnessed to surface.

The well is piped to a gas processing unit. From there, it is routed to a production area that contains four 400-barrel production fluid holding tanks. Annular pressures read on the gauges provided appeared to be 110 pounds in between the outside of the 9.625-inch diameter casing and the inside of the 13.375-inch diameter casing and 0 pounds between the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing. The production records within RBDMS are currently updated to 2017.
1665099084 2015-12-16 Preliminary Restoration No Violations Vegetation has been established at this well site. Erosion and sediment controls are in place and functioning properly. Preliminary Restoration for this site has passed.
1998200266 2018-07-31 Production Wells No Violations Status check inspection found that there are 4 wells present and in operation on the well pad. The pad surface and lease road is of stone construction and in good repair. The pad entrance is gated and locked with signage posted at the end of the lease road along with identification on each well head, the production tanks are also hazard labeled. Master emergency shut down well control devices are situated at the entry point of the well pad as well as at the production fluid storage area and at the well head and gas processing areas. The well head is equipped with a lower A section and upper B section, riser pipes with gauges installed are present on both sections to monitor annular pressure. The pressure on the 13.375 (A) section which is the pressure between the 13.375 and 9.625 diameter casing strings is 230 pounds per square inch (psi) The pressure on the 9.625 well head (B) section which is the pressure between the 9.625 and 5.5 diameter casing strings is 250 psi. The 5.5 diameter casing pressure is 900 psi. The well head assembly is equipped with a tubing head assembly and 2 manual master valves along with a emergency shut down device in the sales line downstream from the manual master valves. The sales line pressure is zero psi due to EQT midstream working on their midstream production facility just off the well pad. Downstream from the well head assembly is the production equipment consisting of a gas processing unit. On the back right corner of the well pad is the production fluid tank farm consisting of (4) steel construction 400 barrel capacity tanks situated inside a polymer lined steel containment dike area. The well pad is protected by an earthen berm and there are fire extinguishers situated at all critical points on the well pad.
