0832261397 2018-06-21 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5 inch diameter production casing that appeared to be open flowing. Annular pressure monitoring was in place, the 9.625 inch diameter intermediate casing gauge read 1200 pounds per square inch, the 13.375 inch diameter surface casing gauge read 340 pounds per square inch. Production equipment included a Line Heater, Sand Trap, vertical fluid separator, and heating coil. There are 2- 500 barrel stock tanks shared with 34013208930000, 34013208940000, 34013209380000. Identification tags were present, and production was up to date. No violations were found at the time of my inspection.
1023539171 2019-02-21 Preliminary Restoration No Violations Preliminary restoration status check inspection found the well site meets ORC 1509.072 with vegetation and erosion control in place. The well pad perimeter is protected by an earthen berm with an 8 chain link fence on the perimeter berm. Signage is posted at the lease road entrance point and on each well head as well as the pad entrance. 4 wells are present on the pad with annular pressure monitoring in place on the well head A&B sections. The production fluid storage consists of (2) steel 500 barrel tanks situated inside a polymer lined steel containment dike area. 4 gas processing units are also present on location. A mid stream pipeline operator identified as Mark West has a station on the left rear corner of the well pad.
