0283362850 2018-08-23 Production Wells No Violations On the day of my inspection I witnessed that the well was currently using flow back equipment including a sand trap and choke manifold system, the sand trap was connected to 3 500-barrel mobile fluid storage tanks. The well head was equipped with 5.5-inch diameter production casing and was open flowing through a manual master valve. Annular pressure monitoring was in place but had no digital readouts available for reference, there were annular pressure bleed down lines on both the 9.625-inch diameter casing, and 13.375-inch diameter casing, at the time of my inspection the 9.625-inch diameter casing valve was closed, and the 13.375-inch diameter casing valve was open. Production equipment included a gas processing unit and 6 400-barrel production fluid tanks that were shared with the other wells on the pad. There was secondary containment around the production fluid tanks and gas processing units. Identification tags were present, and production reports were current.
0367916076 2018-04-24 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5 inch diameter production casing and was open flowing with valves in the open position. Annular pressure monitoring was in place but had no digital readouts available for reference. Production equipment included a sand trap, a Gas Processing unit, line heater, and 6- 400 barrel purge tanks shared with 3401320914, 3401321141, 3401321147. Roustabout crew was currently on location and installing lines for annular pressure release. Identification tags were present and production was up to date.
0426350696 2015-07-06 Casing String Witnessed No Violations On this day Express Energy had reached a total depth of 100. The 30 casing was ran and cemented into the 36 open hole. Express Energy was on site to cement the casing with 275 sacks of Class A cement. Cement to surface was established. See cement ticket for details.
0824596906 2018-04-23 Production Wells No Violations The well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit. From there, it is routed to a production area that contains six 400-barrel production fluid holding tanks. Annular pressures readings supplied by the company were 915 pounds in between the outside of the 9.625-inch diameter casing and the inside of the 13.375-inch diameter casing and 238 pounds between the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing. The production records within RBDMS are currently updated to 2017.
0825101652 2019-03-25 UrbanProduction Wells No Violations Status check inspection found a 3rd party contractor identified as Shale Pro on location and currently on site installing the production tree assembly on the well head B section. The company man on site identified as Tracy Vanscoy indicated the company recently had 2.875 production tubing installed on all 4 wells on the pad. The wells were previously produced through the 5.5 production casing. Production equipment on site includes a production facility consisting of (6) 500 barrel steel tanks situated inside a polymer lined steel containment dike area. Also present is (4) gas processing units. Proper well identification is posted at the well pad entry point and tank battery, once the well heads are assembled the identification will be put back up on the wells. The lease road and well pad surface is stone construction and in good repair, no earthen berm is present. Also noted on 11/03/2016 the said Company XTO Energy submitted a waiver to the Division asking for a variance pertaining to fencing, screening and landscaping, the language pertaining to the waiver is found in OAC 1501:9-9-05.

1753037383 2015-07-11 Casing String Witnessed No Violations On this day First Class Rig #1 had reached a total depth of 439. The 18.625 casing string was ran and cemented into a 26 open hole to a depth of 397. Nabors (C and J) Cementing was on site and cemented the casing with 890 sacks of Class A cement. Cement to surface was established following top out job. See cement ticket for details.
