0495143945 2018-03-16 Production Wells No Violations The well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit. From there, it is routed to a production area that contains four 400-barrel production fluid holding tanks. There were also three 500-barrrel frac tanks on site. Annular pressures read on the gauges provided appeared to be 395 pounds on between the outside of the 9.625-inch diameter casing and the inside of the 13.375-inch diameter casing and 1150 pounds between the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing . The production records within RBDMS are currently updated to 2017.
1054466893 2015-08-25 Completion Testing No Violations Notification was given by the operator that completion operations are aticipated to commence this Friday.
1083639153 2018-10-23 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing that was equipped with a plunger lift system. At the time of my inspection annular pressure monitoring was in place and the 9.625-inch diameter casing was at 60 pounds per square inch, the 13.375-inch diameter casing pressure was 330 pounds per square inch. Production equipment included an individual Sand Trap, and Gas Processing Unit. As well as 4 400-barrel steel production fluid tanks, and a 50-barrel production fluid tank that were shared with the other producing wells on the pad. There was secondary containment around the production fluid tanks. The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location. Identification tags were present, and production was up to date.
1418063110 2014-12-04 Casing String Witnessed No Violations On the date of inspection I am on site to witness the cementing of the conductor pipe. I was informed by Gulfport Energy consultant, Nick Berelsman, that Rocky Mountain Drilling had drilled a 32 inch diameter hole to a depth of 110 feet. Rocky Mountain then set 110 feet of 26 inch diameter pipe weighing 102.63lbs/ft on bottom. Stingray Cementing LLC broke circulation and pumped 254sk of Class A cement, 15.6ppg, 1.21 yield, 3% CaCl2, and 0.25lbs/sk of celloseal. Stingray displaced cement with 62bbl of fresh water. 15bbl of cement returned to surface and stayed at surface while I was on site.
1425165370 2015-02-28 Casing String Witnessed No Violations On site to witness cement job on 18.625 casing and stingray squeezed 200 sacks of Class H cement into perforations and owner will preform another squeeze job later tonight or early morning.

Note: 378 of casing was ran and original job was pumped and cement locked up when contractor released plug and they were unable to pull casing and job was calculated on 60% cement excess.

1425907913 2015-03-09 Drill / Deepen / Reopen Violations Noted The operator was negligent in providing proper notification of the running and cementing of the 13.375 casing string. There is no further action required pertaining to this compliance notice, however Gulfport Energy shall follow the requirements provided in the Ohio Revised Code related to all aspects of drilling and completion including but not limited to the notification parameters.

1893643343 2015-09-21 Preliminary Restoration No Violations On the day of my inspection completion operations were finalized and the rig down and clean process was underway. The erosion and sedimentation controls were of satisfactory condition and proper vegetation had been established. The preliminary restoration for this well has been completed.
