0847605497 2018-11-29 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well site meets ORC 1509.072 with erosion control in place and vegetation established. Previously a slip occurred on the west side of the well pad shortly after the initial excavation construction phase, this area was inspected and no earthen movement was observed. The lease road and well pad surface is stone construction and in good repair. The well pad entrance does have a gate present. 3 wells are present on the well pad with the said well being equipped with a lower A and upper B section, attached to the A&B section well head ports is 2 diameter riser pipes that are equipped with valves and gauges for annular pressure monitoring purposes. The 13,375 well head ( A) section gauge reads 375 pounds pressure. The 9.625 well head (B) section gauge reads 1,250 pounds pressure. The 5.5 diameter casing internal pressure is 500 pounds pressure. The well head assembly does have a tubing production tree and the flow line pressure down stream from the well head choke is 200 pounds pressure. The well is producing into a gas processing unit. The production fluid storage area consists of (4) steel construction 400 barrel fluid capacity tanks and one 50 barrel tank situated inside a polymer lined steel containment dike area.

The reason for the on site meeting was to review 2 separate earthen slips on the outward slopes of the well pad. The company had submitted material modifications and is working with the ODNR Engineering staff to address the slips. There is 3 wells present on the pad that are producing, the production equipment and production flow lines are on the opposite side of the pad from the earthen slip. The wells are producing.
