0773917779 2018-08-21 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a 4-string casing design and was in normal production. Annular pressure monitoring was in place on both the 9.625-inch diameter casing gauge read 1225 pounds per square inch, the 13.375-inch diameter casing gauge read 565 pounds per square inch. Production equipment included an individual Gas Processing unit, and 6- 400-barrel production fluid tanks shared with the other producing wells on the pad. The well pad and all equipment were within earthen containment with secondary containment around the production tanks. There was also a large line heater on location and foam and defoamer tanks near the well head. There were multiple Emergency Shutoff Devices on location, Identification tag were present, and production was current.

1333928671 2017-10-26 Production Wells No Violations This well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit and then to a production area that contains six 400-barrel production fluid holding tanks. Production records for this well are current to 2017 within RBDMS.
1417440629 2014-11-17 Drill / Deepen / Reopen No Violations On this day Falcon 23 had begun to drill the 28 open hole to run the 24 casing. No violations noted.
1420064431 2014-12-29 Casing String Witnessed No Violations Inspected 9.625 casing and rig crew was preparing to trip out of hole to run casing and they were at a depth of 8610 and they plan on running 8561 of casing and cement it in place with 1547 sacks of cement. 1162 sacks of 11# cement and 385 sacks of Class A 15# cement and casing weight was 40# per ft. and they ran guide shoe and float collar and several centralizers.

1687823512 2014-11-14 Casing String Witnessed No Violations On the day of my inspection Well Service Group had drilled the 36 open hole and 30 casing was ran and cemented. See cement ticket for details. No violations noted.
1968641033 2014-11-12 Drill / Deepen / Reopen No Violations On this day the well had been spudded.
2027967707 2018-08-07 Production Wells No Violations Status check inspection found the well pad surface and lease road has a gravel surface and is in good repair. There are 6 wells present on the pad all in production with identification posted at each well. The said well is equipped with a lower A section and upper B section with 2 diameter riser pipes present at the well head and equipped with valves and manual pressure gauges to monitor annular pressure. The 13.375 x 9.625 diameter casing pressure was 690 pounds per square inch (psi) the 9.625 x 5.5 diameter casing pressure was 1,257 psi. The 5.5 diameter production casing internal working pressure was 1,200 psi and the production flow line pressure was 1,065 psi. The well is selling off the 5.5 production casing. There are 4 gas processing units on location along with a large glycol heater. At the left front corner of the well pad is the production fluid facility consisting of 6 steel construction 400 barrel capacity production tanks located inside a polymer lined steel containment dike area. There is also a midstream station at the rear of the well pad. Fire extinguishers and emergency shut down control points are present at all the critical areas of the well pad. The well pad perimeter is protected by an earthen berm.
