0060060249 2018-05-02 Production Wells No Violations The well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit. From there, it is routed to a production area that contains two 500-barrel production fluid holding tanks. Annular pressures read on the gauges provided appeared to be 760 pounds in between the outside of the 9.625-inch diameter casing and the inside of the 13.375-inch diameter casing and 1600 pounds between the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing. The production records within RBDMS are currently updated to 2017.

0743014441 2016-09-21 Drill / Deepen / Reopen No Violations On this day I was notified by the company man on site that the Drilling rig on the well has reached the total proposed depth of 19,892 and will condition the hole before pulling the drill string and running the 5.5 production casing. The rig will then move to the Lee pad.
1490483696 2015-01-13 Drill / Deepen / Reopen No Violations On the date of inspection, Express Energy Services were in the process of drilling the conductor hole. They had trouble with the hole collapsing and had filled the portion that was drilled with ready-mix cement for stability.
1557314142 2017-03-27 Preliminary Restoration No Violations Preliminary restoration status check inspection found the well drilling and stimulation process is now complete. The well head has had the completion plugs drilled out and has production tubing installed. The production tree portion of the well head has 2 production flow lines attached with 2 solenoid controlled auto or manual choke valves on the tubing flow cross head. The B section has no flow lines attached and there is annular monitoring gauges installed on all annular well head sections. The well cellars on the 3 existing wells on the pad are free of standing fluid. A pressure testing service is on location pressure testing all production flowlines downstream from the well head. There is 3 gas processing units on location along with line heaters. At the production fluid storage facility a total of 2 steel tanks are present that are capable of holding 400 barrels capacity each. The well pad sits at 700 feet in elevation next to Captina Creek and has an earthen berm present as primary containment with the perimeter drain valves closed. The company man on site indicated they were to begin flow back operations in the next 48 hours and the drain valves for the pad would remain closed during those operations. The location perimeter is fenced by a 8 chain link fence and the location entrance point has signage posted. With emergency shut down devices at the well pad entrance The area affected by the well pad construction has erosion control in place and vegetation established.

1906864337 2015-01-14 Administrative Inspection No Violations I received a number of calls from American Energy Utica (AEU) drilling superintendent, Vernon Gallo. Due to the repeated collapsing of the conductor hole drilled by Express Energy Services, AEU plans to plug and abandon the portion of the conductor hole that was drilled. AEU initially had plans to submit an emergency permit to spud a new well to take the place of the No. 5, but had finally decided to put a hold on this well pad indefinitely. Vernon scheduled Universal Well Services to plug and abandon the hole on 1-15-15 at approximately 11AM.
